The New ICP Minimally Invasive Method Shows That the Monro–Kellie Doctrine Is Not Valid

Abstract: The Monro-Kellie doctrine states that the interior of the cranium is formed of three main components: blood, fluid and cerebral parenchyma. An increase in the volume of one or more components may increase the intracranial pressure (ICP). This doctrine also affirms that the skull cannot be expanded after the closure of the fontanels. Monro and Kellie's theory has been perfected during the last two centuries. This study leads to a new contribution that proves that even adults' consolidated skulls present volumet… Show more

“…This information was quickly Advances in Cerebral Monitoring absorbed into CSF flow diversion following the first ventricular puncture in 1881 by Wernicke. 38 The modern support for ICP monitoring is due in large part to the assumption that the Monro-Kellie doctrine accurately describes intracranial pathology and that pressure within the ventricles reflects pressure throughout the intracranial space. Recently, several investigators have questioned whether the Monro-Kellie doctrine fully accounts for intracranial dynamics.…”

“…Recently, several investigators have questioned whether the Monro-Kellie doctrine fully accounts for intracranial dynamics. [37][38][39] ICP should provide the practitioner with insight to a relative increase or decrease of one or more of these contents. 40 There are neither clinical trial data nor class I evidence to support ICP as a predictor of outcome.…”
