The New News about Lithium: An Underutilized Treatment in the United States

Abstract: Lithium use for the treatment of mood disorders remains quite low, particularly in the United States compared with some European countries. Mogens Schou pioneered the study of lithium for prophylaxis of the recurrent mood disorder and encouraged its greater use. In an effort to further address the appropriate role of this drug, the multiple assets of lithium beyond its well-known antimanic effect are reviewed, and a brief summary of its side effects is outlined. It appears that lithium has positive effects in … Show more

“…Despite its proven efficacy [159], there are reports when lithium administration determined side effects such as thirst and excessive urination, nausea, diarrhea, tremor, weight gain, sexual dysfunction, dermatological effects, and long-term effects on the thyroid gland, kidneys, and parathyroid glands [160]. However, recent studies challenge this negative perception of lithium administration, stating that intoxication occurs only when the concentration is higher than 1.5 mmol/L, and its main adverse effects can be properly monitored and managed [11,[161][162][163].…”

“…Willingness to consistently take the drug trumps other considerations, and one can hope that some of lithium's many other assets would occur at these lower levels, including its anti-suicide effects, increases in hippocampal and cortical volume, prevention of cognitive deterioration, increasing the length of telomeres, and increasing production of stem cells. 16 Lower than usual lithium levels also can have a role at a first or second episode or in a well interval. Gradual dose titration when the agent is first started is can be valuable, as people tend to generalize and extrapolate their experiences, and the first exposure influences people to believe that they do or do not tolerate medicines in general.…”
