Case summary A 9-year-old, spayed, female domestic shorthair cat presented with an open wound approximately 1 cm in size with exposure of the left subcutaneous ureteral bypass (SUB) shunting port that was placed approximately 11 months before presentation. Primary closures were attempted twice before local wound management with omentalisation and repositioning of the port. The exposed port was lavaged topically with a polyhexanide and propylbetaine wound irrigation solution before omentalisation and repositioning, resulting in successful retention of the implant. Five months after revision and omentalisation, there was complete coverage and healing of the wound. Relevance and novel information Adequate topical treatment, repositioning and omentalisation could be a successful treatment option for the uncommon complication of SUB shunting port extrusion secondary to resistant local infection originating from the urinary tract.

Surgical Treatment of Lumbosacral Discospondylitis with Gentamicin-Impregnated Polymethylmethacrylate Cement and Omentalization

Objective The aim of this study was to describe a novel technique using a gentamicin-impregnated polymethylmethacrylate (PMMA) plug for the surgical treatment of lumbosacral discospondylitis with concomitant instability. Clinical Report A 7-year-old male German Shepherd dog with lumbosacral (LS) discospondylitis and previously diagnosed with degenerative lumbosacral disease underwent ventral slot and distraction of the L7 to S1 intervertebral space with a gentamicin-impregnated PMMA plug. The lumbosacral joint was accessed via the abdomen. Samples were collected for bacterial culture and sensitivity, fungal culture and histopathological examination. The surgical site was omentalized. Long-term oral antimicrobials were administered. Results Clinical improvement was seen immediately after surgery. Complete return to previous activity level was observed 12 months after surgery. Follow-up radiographs 18 months after the procedure revealed no implant migration. Clinical Significance The gentamicin-impregnated PMMA plug in addition to the surgical debridement seemed to be an effective way for short-term distraction, potentially contributing, along with the appropriate antimicrobial therapy and analgesia, to alleviation of pain immediately after surgery and providing for a good short-term outcome, in this clinical case. The use of cement plugs as sole devices in the LS joint warrants further study.

A male entire boxer was presented for investigation of haematuria and urinary incontinence. Rectal examination identified bilateral perineal herniation and enlargement of the prostate. Ultrasonographic and radiographic findings were consistent with diffuse prostatic enlargement and intrapelvic paraprostatic cyst formation, with mineralisation of the cyst wall. Omentalisation of the cyst was carried out in conjunction with colopexy and vas deferens pexy. The dog was represented two weeks later with severe dysuria, urinary retention, tenesmus and recurrent haematuria. A combined abdominal and targeted perineal surgical approach was performed to allow omentalisation of the cyst from the region of the bladder to the perineal fossa. One-year follow-up confirmed resolution of clinical signs. A combined abdominal and perineal technique for omentalisation of intrapelvic paraprostatic cysts is a novel technique, which can be used when faced with reduced surgical exposure within the pelvic cavity.

The Omental Pedicle Flap in Dogs Revised and Refined: A Cadaver Study

Abstract: When lengthening the omentum is necessary to reach extra-abdominal structures, the omental pedicle flap based on the splenic artery appears to preserve the omental vascular supply. These observations warrant further clinical trials to evaluate this new omtental flap technique in vivo.
