The pathogenesis of traumatic coagulopathy

Abstract: SummaryOver the last 10 years, the management of major haemorrhage in trauma patients has changed radically. This is mainly due to the recognition that many patients who are bleeding when they come in to the emergency department have an established coagulopathy before the haemodilution effects of fluid resuscitation. This has led to the use of new terminology: acute traumatic coagulopathy, acute coagulopathy of trauma shock or trauma‐induced coagulopathy. The recognition of acute traumatic coagulopathy is impo… Show more

“…Since the publication of the fifth National Audit Project (NAP5) regarding accidental awareness during general anaesthesia (AAGA) [1], there have been a number of editorials and commentaries on the implication of the study's results for specific areas of practice, such as cardiac anaesthesia [2], obstetric anaesthesia [3], and even the administration of sedation [4]. One of the authors (JCA) became interested in awareness and depth of anaesthesia monitoring after being involved in some lowlevel research as a senior registrar in the last century!…”

“…The original guideline for the management of bleeding trauma patient was published in Critical Care in 2007 (8), and updated in 2010 (9), 2013 (10) and 2016 (11). Over the last 10 years, clinical management has been changed from maintaining an adequate circulating volume and oxygen carrying capacity first, then dealing with coagulopathy to hemostatic resuscitation as early as possible (5). In the recent publication, Rossaint and colleagues presented 39 recommendations on initial resuscitation and prevention of further bleeding, diagnosis and monitoring of bleeding, tissue oxygenation, type of fluid and temperature management, rapid control of bleeding, initial management of bleeding coagulopathy, further resuscitation and guideline implementation and quality control (11).…”
