The Pathologic Findings of Skin, Lymph Node, Liver, and Bone Marrow in Patients With Adult-Onset Still Disease

Abstract: Adult-onset Still disease (AOSD) is characterized by fever, skin rash, and lymphadenopathy with leukocytosis and anemia as common laboratory findings. We investigated the characteristic pathologic findings of skin, lymph node, liver, and bone marrow to assist in proper diagnosis of AOSD.Forty AOSD patients were included in the study. The skin (26 patients), lymph node (8 patients), liver (8 patients), or bone marrow biopsies (22 patients) between 1998 and 2013 were retrospectively analyzed. AOSD patients were … Show more

“…Liver abnormalities are common in patients with AOSD, occurring in 65% of cases . The histological findings are similar to those of nonspecific reactive hepatitis . Inflammatory cytokines, such as IL‐1, IL‐18, TNF, and IL‐6, are thought to play an important role in the liver enzyme elevation seen in both FMF and AOSD …”

“…Increased level of liver enzymes, which is included in the minor criteria by Yamaguchi et al., is common in AOSD ( 3 ). Histology of AOSD liver is often characterized by mild hepatitis accompanied by neutrophilic infiltration ( 8 ). Although uninvestigated in the liver of AOSD, NETosis has been described to have a potential role in many other hepatic inflammatory conditions, i.e., by favoring the influx of monocyte-derived macrophages and enhancing production of inflammatory cytokines ( 26 ).…”

“…More than 80% of AOSD patients present with neutrophilic leukocytosis during the acute flare (2). Histology of the rash, lymph nodes, and liver are also characterized by neutrophil infiltration (8), suggesting that neutrophils are critical effector cells in AOSD.…”

“…Skin biopsy of the evanescent rash revealed hyperkeratosis and focal parakeratosis of the stratum corneum. Numerous apoptotic keratinocytes were appreciated in clusters and singly within the stratum corneum, suggestive of Still's Disease [4, 5]. …”
