Background: Intrauterine contraceptive device (IUCD) is one of the most widely used methods of reversible birth control. Consequently, the existence and consequences of insertion, removal and associated infection have received little attentionObjective: To identify the type and to determine the incidence of infection associated with IUCD users through screening by culturing endocervical swabs.Methods: This is a descriptive, cross sectional, facility based study conducted in family planning clinics in Khartoum. This study was carried out on 35 ladies, attending the clinics during the period of study, using IUCD, presented for follow up, complaining or for removing the IUCD, who fulfilled the criteria of selection after obtaining an informed consent.Results: The incidence of positive culture swab among users was 65.0%. Among the study population, the age distribution was found to be varying between 24 and 39 years, taking a range of 4 years between groups of study population. Regarding complaint, vaginal discharge was found among 32 (91.4%) users, intermenstrual bleeding was present among 15 (42.9%) users, menorrhagia has been present in 11 (31.4%) users, abdominal discomfort was noticed among 6 (17.1%) users, and postcoital bleeding was present in 5 (14.3%) users. Result of swab culture shows that, 12 women (34%) of users culture of no growth, while 9 of them Staph.spp positive culture, there were 3 women (8.6%) users with culture result of E.coli, and 3 women (8.6%) users with culture result of candida albicans, there were 2 women (5.7%) users with culture result of coliform and other 2 women (5.7%) users with culture positive for trichomonas vaginalis.Conclusion: The study concluded that incidence of positive cultures among IUCD users was 65.0% and about two third of female in the family planning clinics had mild to moderate levels of infection. The most common isolated organisms among IUCD users in this study were Staph spp, E.coli and Candida .The study also showed high rate of removal of IUCD copper type and most of removal due to dyspareunia.

Background: Fibroids are the most common pelvic tumors in females, affecting approximately 30%-50% of women of fertility age. Intrauterine devices (IUDs) are widely used in the world. Copper-IUDs which are one of the common causes of intrauterine chronic inflammation might create a suitable microenvironment for fibroid growth. This study aimed to find an answer to investigate whether there is an association between fibroids and copper (Cu) T-IUD use. Methods: This cross-sectional study was conducted with 788 participants. The participants were divided into two groups based on fibroid presence (study group) or absence (control group). For this study, usage of IUDs was defined as Cu T-IUD use for at least one year. Medical and obstetric history, oral contraceptive (OC) and/or IUD use and duration of use, as well as smoking, were questioned. Results: In participants who had fibroids, IUD use was found to be statistically significantly higher (55.5% vs 43.4%) (P = 0.001). In addition, according to multiple logistic regression analysis, having fibroids was found to be significantly related to age, number of abortions, smoking, and duration of Cu T-IUD use. A statistically significant correlation was not found between fibroid diameter, parity, and duration of OC use; however, a statistically weak correlation was found between fibroid size and duration of Cu T-IUD use. Conclusions: This study points to the association of Cu T-IUD use, inflammation, and fibroids. Our results can provide a steppingstone for the development of additional studies investigating this hypothesis. If inflammation is the preliminary event and copper IUDs cause inflammation, preventive health strategies may be implemented to lessen the possibility of fibroid development.

Objective: The aim of the study was to provide an overview to infectious and reactive cytological effects of intrauterine devices, which are one of the most widely used methods of contraception, with the results we obtained in the Mardin region. Material and Method:We evaluated together the pap smears of the 300 intrauterine device (+) and 300 intrauterine device (-) patients sent to the pathology department in the period of 2010-2011. Genital infection rates and frequency of reactive-dysplastic cellular changes were statistically compared between the groups together with the literature data. Results:In the intrauterine device (+) group, 2 (+) and more severe inflammation was observed in 66.3% of cases. Bacterial vaginosis (26%), candida (12%) and trichomonas vaginalis (8%) were recorded as the most common infectious agents, followed by actinomyces (4%). Between the groups of intrauterine device (+) and (-), no significant difference was observed in terms of the incidence of squamous cell abnormalities, except ASC-US (p=0.02). In the intrauterine device (+) group, the presence of atypical glandular cells and reactive findings was significantly higher than the control group. After the removal of the intrauterine device, 61 cases that had control smears showed regression, with a rate of %75.4 (n=46). Conclusion:The local irritative and inflammatory effect of intrauterine devices basically causes reactive and regenerative changes mostly in glandular cells. Intrauterine devices disrupt the genital flora and significantly increase the frequency of genital infection by creating a foreign body reaction. Bulgular: Rahim içi araç (+) hasta grubunda 2 (+) ve üzerinde inflamasyon yoğunluğu gösteren olguların oranı %66 olarak izlenmiştir. En sık enfeksiyon etkeni olarak, bakteriyel vajinozis (%26), candida (%12) ve trikomonas vajinalis (%8) kaydedilmiştir. Aktinomiçes ise %4 oranında izlenmiştir. Rahim içi araç (+) hastalarda, (-) olanlara göre ASC-US ve AGC görülme sıklıkları anlamlı olarak yüksek izlenmiştir (p=0,02, p<0,01). Ayrıca skuamöz metaplazi, tubal metaplazi, sitoplazmik balon vakuollü hücreler, HGSIL'i taklit eden izole hücreler gibi reaktif bulguların izlenme sıklığı rahim içi araç (-) gruba göre anlamlı oranda yüksek izlenmiştir. Rahim içi araç çıkartıldıktan sonra takip smirleri olan 61 olguda %75,4 (n=46) oranında bulgularda gerileme saptanmıştır.Sonuç: Rahim içi araç lokal irritatif ve inflamatuvar etkisi ile temel olarak glandüler hücrelerde belirgin reaktif ve rejeneratif değişikliklere neden olmaktadır. Yabancı cisim ortamı yaratarak genital florayı bozmakta ve genital enfeksiyon sıklığını belirgin ölçüde artırmaktadır. Kendi bölgemizden elde edilen sonuçlar da bu bulguları destekler yöndedir.
