To the EditorWe read the paper by Bouley et al. with interest, since we have been following the development of so-called IntralipidB-Amphotericin formulations for some years, and have published a number of articles in this general area. Consequently we were most concerned to see that the paper by Bouley et al. misquoted our recent work.It is important to realise that there are two rather distinct schools of thought concerning the formulation of amphotericin emulsions. The authors of the paper under discussion produced emulsion formulations by simply mixing reconstituted Fungizone@ (amphotericin B solubilized in sodium deoxycholate) with IntralipidB. We have previously shown that in such mixtures the amphotericin precipitates almost immediately, and that consequently the therapeutic value of such formulations is unclear.*The emulsion formulations made at Nottingham by our group are manufactured de novo in a specific manner to ensure that the amphotericin is associated with the emulsion droplet. We believe this is essential if the full potential of an emulsion delivery system is to be achieved. We have demonstrated by fluorescence-quenching experiments that the amphotericin is present in the oil/water interfacial layer in these formulation^.^ This latter reference is quoted by Bouley et al. as no. 10 in their list.We were therefore most surprised that Bouley et al.' cite reference 10 as follows: 'In an IntraIipidB formulation, the AMB seems to be incorporated into the phospholipid monolayer which forms the interface of the lipidic dropletsI0' (p. 486). Reference 10 does not support IntralipidB formulations in this way; indeed our previous work shows exactly the opposite effect;* viz, that precipitation is the usual fate of emphotericin B in such formulations. Furthermore, we have seen no evidence from the French workers that, in their formulations, there is any association at a molecular or colloidal level between amphotericin and the emulsion droplets. We would be most interested to see such evidence if it can be provided.
