The Persisting Significance of Oligoclonal Bands in the Dawning Era of Kappa Free Light Chains for the Diagnosis of Multiple Sclerosis

Abstract: The latest revision of the McDonald criteria of 2017 considers the evidence of an intrathecal immunoglobulin (IgG) synthesis as a diagnostic criterion for dissemination in time in multiple sclerosis. While the detection of oligoclonal bands is considered as the gold standard, determination of kappa free light chains might be a promising tool as a less technically demanding and cost saving method. However, data on the direct comparison between kappa free light chains and oligoclonal bands are limited and no stu… Show more

“…Various studies indicate that high CSF concentrations of kappa FLCs seem to be a more sensitive marker of intrathecal IgG synthesis compared with the finding of CSF-restricted OCBs [5][6][7][8][9][10]15,[18][19][20], although recent studies point to a higher sensitivity of OCB detection [21] or to an equal performance of both tests [22].…”

“…Patients were included when they were either newly diagnosed with MS according to the 2017 McDonald criteria or converted to MS during follow-up. Some of these patients were previously investigated with a focus on different other aspects [14][15][16][17][18]. A total of 107 patients were included.…”
