The physiological consequences of different distributions of diffuse pleural thickening on CT imaging

Abstract: Our study shows an incremental reduction in the forced vital capacity and total lung capacity in DPT without CPA obliteration, unilateral and bilateral DPT when compared with a matched control group. Advances in knowledge: Different distributions of DPT including no CPA obliteration can cause respiratory impairment, with bilateral DPT being the worst affected.

“…Duneesha et al. reported reductions of the vital capacity and total lung capacity in DPT patients, without cost-phrenic angle obliterance [9] . Asbestos-induced respiratory function and pulmonary lesions require >10 years to develop [8] , and heart disease and mediastinal disease can be treated with cardiac surgery or radiation therapy [10] , which may contribute to the progression of pleural disease.…”

“…The criteria for compensation and Asbestos Health Damage Relief in Japan do not consider the dullness of costophrenic angles, which is a British criterion, but according to Fonseka et al 7 ) , even without the dullness of costophrenic angels, %VC reduced to 79.5% in unilateral diffuse pleural thickening and 66.7% in bilateral cases, leading to a respiratory compromise within the range of diffuse pleural thickening, which is the Japanese criterion. In addition, Singh et al 8 ) reported that the movements of the diaphragm and lower pleura greatly affected the respiratory function of diffuse pleural thickening patients.…”

“…Diffuse pleural thickening may lead to significant respiratory disability. In subjects with diffuse pleural thickening, forced vital capacity and single breath diffusing capacity are considered to be lower in comparison to subjects without this disorder [35][36][37].…”

“…From the diagnostic point of view, a chest radiograph is used as a standard method for detecting diffuse pleural thickenings; however, also in this case, HRCT scans are far superior to any other method [20,37,38].…”
