The Presence of Sodium Nitrate in Generator Eluate Decreases the Radiochemical Purity of 99mTc-Sestamibi

Abstract: A high radiochemical purity (RCP) is recommended for radiopharmaceutical compounds used in the clinical practice of nuclear medicine. However, some preparations of 99m Tc-sestamibi contain excess impurities (.6%). To understand the origin of these impurities, we investigated the effect of sodium nitrate on the RCP of sestamibi preparations by testing eluates from 3 commercially available 99m Tc generators. Methods: The sestamibi kits (Stamicis) were reconstituted with 99m Tc eluate from nitrate-containing wet-… Show more

“…The experimental evidence of non-thermal EMF exposure shows increases in the permeability of the erythrocyte membrane, effects on the BBB, and increases in calcium ion efflux (20). The 99m Tc-MIBI uptake consists of radiotracer passive distribution across plasma and mitochondrial membranes (21,22). On the other hand, the uptake of 99m Tc-MIBI also changes with the intracellular concentration of Na + , indicating that 99m Tc-MIBI behaves like Na + during its uptake (23).…”

“…This effect can cause an undesired uptake of [ 99m Tc]TcO 4 − into the thyroid, but also into the salivary and gastric tissues, compromising image quality and delivering an unjustified radiation dose to the patient (Hingorani et al 2010 ). The efficacy of radiolabelling of the nanocolloidal HSA depends on the size distribution of the particles, however, any excipients such as buffers, inorganic ions, or large amount of glucose present within the kit, as well as the quality of the generator eluate may all play a role in the speciation of 99m Tc (Persico et al 2015 ; Metaye et al 2012 ; Waight et al 2011 ).…”
