1467Objective: Inflammation and metabolism are closely interlinked. Both undergo significant dysregulation following surgery for congenital heart disease, contributing to organ failure and morbidity. In this study, we combined cytokine and metabolic profiling to examine the effect of postoperative tight glycemic control compared with conventional blood glucose management on metabolic and inflammatory outcomes in children undergoing congenital heart surgery. The aim was to evaluate changes in key metabolites following congenital heart surgery and to examine the potential of metabolic profiling for stratifying patients in terms of expected clinical outcomes. Design: Laboratory and clinical study. Setting: University Hospital and Laboratory. Patients: Of 28 children undergoing surgery for congenital heart disease, 15 underwent tight glycemic control postoperatively and 13 were treated conventionally. Interventions: Metabolic profiling of blood plasma was undertaken using proton nuclear magnetic resonance spectroscopy. A panel of metabolites was measured using a curve-fitting algorithm. Inflammatory cytokines were measured by enzyme-linked immunosorbent assay. The data were assessed with respect to clinical markers of disease severity (Risk Adjusted Congenital heart surgery score-1, Pediatric Logistic Organ Dysfunction, inotrope score, duration of ventilation and pediatric ICU-free days). Measurements and Main Results: Changes in metabolic and inflammatory profiles were seen over the time course from surgery to recovery, compared with the preoperative state. Tight glycemic control did not significantly alter the response profile. We identified eight metabolites (3-d-hydroxybutyrate, acetone, acetoacetate, citrate, lactate, creatine, creatinine, and alanine) associated with surgical and disease severity. The strength of proinflammatory response, particularly interleukin-8 and interleukin-6 concentrations, inversely correlated with PICU-free days at 28 days. The interleukin-6/interleukin-10 ratio directly correlated with plasma lactate. Conclusions: This is the first report on the metabolic response to cardiac surgery in children. Using nuclear magnetic resonance to monitor the patient journey, we identified metabolites whose concentrations and trajectory appeared to be associated with clinical outcome. Modulation of the human cytokine response by interferon lambda-1 (IFN-λ1/IL-29)

The interferon lambda family (IFN-l1/2/3) is a newly described group of cytokines that are related to both the type-1 interferons and IL-10 family members. These novel cytokines are induced during viral infection and, like type-1 interferons, display significant anti-viral activity. In order to understand their function in more depth, we have examined the ability of IFN-l1/IL-29 to regulate cytokine production by human immune cells. Whole peripheral blood mononuclear cells (PBMC) exposed to IFN-l1 specifically upregulated IL-6, -8 and -10 but there were no visible effects on TNF or IL-1. This response was produced in a dose-dependant fashion and was inhibited by IL-10. Examination of purified cell populations isolated from PBMC demonstrated that monocytes, rather than lymphocytes, were the major IFN-l1-responsive cellular subset, producing IL-6, -8 and -10 in response to IFN-l1. Monocyte responses induced by low-level LPS stimulation were also synergistically enhanced by the presence of IFN-l1. Human macrophages were also shown to react to IFN-l1 similarly to monocytes, by producing the cytokines IL-6, -8 and -10. In conclusion, we have shown that IFN-l1, a cytokine produced in response to viral infection, activates both monocytes and macrophages producing a restricted panel of cytokines and may therefore be important in activating innate immune responses at the site of viral infection.

Interleukin-6 (IL-6) influence on colonic anastomosis healing in rats

Purpose:To observe the effect of pro-inflamatory citokin, Interleukin-6 (IL-6), administration on the healing of colonic anastomosis in rats. Methods: Thirty Wistar rats were divided into 2 groups of 15 animals each: a control group and an experiment group. The animals of the experiment group were submitted to injection of 500µg/kg of IL-6 Recombinant (Sandoz Phamarceutical ® ) subcutaneously 24 and 12 hours preceding the surgery, and the control group, were submitted to the injection of saline solution, using the same volume of solution as well as the way of administration. The rats were submitted to an end to end colonic anastomosis, being sacrificed in the 7 th post-operative day, the site of the anastomosis was evaluated regarding to the adhesions that was quantified and the presence of complications was observed. The colon containing the anastomosis was submitted to a tensiometric study and a sample were sent for histological studies with HE and Sirius Red dye. Results: The macroscopic and the tensiometric study did not demonstrate any significant differences between the control and experiment group. Regarding the histological evaluation, there was a significant reduction of the chronic inflammatory process in the experiment group (p=0,0209) and a lesser granulation area (p=0,0099). The total amount of collagen was similar in both groups, however, the experimental group showed a predominant area of immature collagen (p=0.009). Conclusions: The longer persistence of the acute phase of wound healing in the experiment group, as well as a decreased of the mature collagen type I, demonstrating that the IL-6 was harmful to the colonic healing anastomosis in rats. RESUMOObjetivo: Verificar a influência da administração da citocina pró-inflamatória, Interleucina-6 (IL-6), na cicatrização de anastomoses colônicas em ratos. Métodos: Utilizaram-se 30 ratos Wistar, divididos em 2 grupos, controle e experimento, de 15 animais cada. Os animais do grupo experimento receberam nas 24 e 12 horas precedentes a cirurgia, 500µg/kg de IL-6 Recombinante (Sandoz Phamarceutical ® ) via subcutâneo, os animais do grupo controle, receberam a administração de soro fisiológico nesse mesmo período, pela mesma via. Foram submetidos à anastomose colônica término-terminal. No 7° dia pós-operatório foram realizadas a eutanásia, com quantificação macroscópica das aderências e visualização de complicações. Em seguida realizou-se o estudo tensiométrico, seguido do estudo histológico das lâminas coradas com Hematoxicilina Eosina e Sirius Red. Resultados: O estudo macroscópico e tensiométrico não demonstraram diferenças significativas entre os grupos estudados. Na avaliação histológica, os resultados significativos demonstraram uma diminuição no grupo experimento do processo inflamatório crônico (p=0,0209) e uma menor área de granulação (p=0,0099) presente neste grupo.
