The Rehabilitation of a Runner With Iliopsoas Tendinopathy Using an Eccentric-Biased Exercise-a Case Report

Abstract: Background and Purpose: While there is much discussion about tendinopathy in the literature, there is little reference to the less common condition of iliopsoas tendinopathy, and no documentation of the condition in runners. The iliopsoas is a major decelerator of the hip and eccentric loading of the iliopsoas is an important component of energy transfer during running. Eccentric training is a thoroughly researched method of treating tendinopathy but has shown mixed results. The purpose of this case report is … Show more

“…The last clinical entity that we consider is the iliopsoas-related groin pain, with a prevalence ranging from 2.4 to 32% in sports that require repetitive kicking and sprinting [ 23 , 41 ]. The diagnostic criteria are hip flexor complex tenderness, pain on hip extension stretch (modified Thomas test) and pain on resisted hip flexion in different positions [ 23 , 24 ].…”

“…Iliopsoas tendinopathy presents more as anterior hip pain in younger athletes, especially after a rapid growth spurt (Rauseo, 2017). Examination usually reveals painful hip flexors.…”

“…As for other tendinopathies, most patients benefit from conservative treatment (Johnston et al, 1998). Eccentric exercise is an appropriate method to use in the rehabilitation of iliopsoas tendinopathy, as seen in runners (Rauseo, 2017). Rehabilitation treatment is divided into five steps, according to Nanni et al (Nanni, 2017): pain, swelling, and inflammation reduction; range of motion and extensibility recovery; strength and resistance recovery; coordination and proprioception recovery; specific technical movements and athletic sport-specific parameters recovery.…”

“…They can affect the restoration of the range of motion in individual joints [2][3][4]. They are used in the case of shortened muscles, weakened muscles, restricted joints and lymphatic drainage [1,5,6]. There are also scientific reports that these techniques have a pain-reducing effect in the case of various disorders of the locomotor system [7,8].…”
