The relationship between carotid artery plaque stability and white matter ischemic injury

Abstract: Higher local carotid artery strain has previously been shown to be a characteristic of unstable carotid plaques. These plaques may be characterized by microvascular changes that predispose to intraplaque hemorrhage, increasing the likelihood of embolization. Little is known however, about how these strain indices correspond with imaging markers of brain health and metrics of brain structure. White matter hyperintensities (WMHs), which are bright regions seen on T2-weighted brain MRI imaging, are postulated to … Show more

“…Carotid strain elastography, shear wave elastography and ARFI are ultrasound methods that can assess mechanical properties of the arterial wall. High strain values are associated with softer plaques (those with characteristics of vulnerable plaque; large lipid core, intraplaque hemorrhage, thin fibrous cap) (Shi et al 2008; Shi et al 2009), increased white matter hyperintensities on magnetic resonance imaging (Berman et al 2015), and poor performance on cognitive function testing (Wang et al 2013; Wang et al 2014). Shear wave elastography has been shown to provide information regarding plaque stiffness, with lower Young’s Modulus (YM) associated with vulnerable plaques (Garrard et al 2015).…”

“…However, clinical management is largely determined by the percent stenosis as derived from medical imaging tests (Dempsey et al , 2010; Brott et al , 2011; NASCET, 1999) and therefore does not include comprehensive evaluation of the composition and mechanical properties associated with plaque instability. Our research utilizes structurally-based, noninvasive strain indices as vascular biomarkers for early diagnosis of vascular sites in the carotid at risk for embolization due to unstable plaque (Wang et al , 2016; Wang et al , 2015; Berman et al , 2015; Wang et al , 2014; Rocque et al , 2012; Dempsey et al , 2010). …”

“…Accumulated axial and lateral strain indices were shown to be capable of assessing vulnerability of carotid plaque in human subjects, and were significantly correlated with cognitive impairment (Shi et al 2008; Wang et al 2014), and increases in white matter hyperintensities (WMH) (Berman et al 2015), (bright regions on T2 weighted brain MRI due to cumulative subclinical microvascular injury). We have also recently illustrated that shear strain indices obtained with the inclusion of the adventitial layer demonstrated significant correlations with cognitive function in human subjects (Wang et al 2015).…”
