Background A Case Difficulty Assessment Form was designed for use in endodontic curricula, and to assist practitioners with treatment planning, referral and recording. The aim of this study was to determine how endodontic case difficulty factors influence the operating time of single-visit nonsurgical endodontic treatments under general anesthesia. Methods Data on 198 single-visit endodontic treatments (80 anterior teeth, 43 premolars, and 75 molars) performed under general anesthesia by a specialized practitioner were obtained from 119 special needs patients (mean [SD] age = 30.7 [14.7] years). Total duration of operation was analyzed with relation to demographic and dental factors and American Association of Endodontists (AAE) Case Difficulty Assessment factors. Mann–Whitney U test, t-test, and Kruskal–Wallis test were used to assess relationships between operating time and confounding factors (p < 0.05). Results High difficulty cases required significantly longer time to complete operations than treatments of minimal-to-moderate difficulty regardless of tooth type (p < 0.05). Demographic factors of the patients rarely influenced operating time length. Among variables included in the AAE Case Difficulty Assessment Form, tooth position, crown morphology, root morphology, canal appearance, and periodontal condition were significantly associated with increased operating time (p < 0.05). Conclusions A higher level of case difficulty contributed to increased duration of endodontic treatment under general anesthesia indicating that Endodontic Case Difficulty Assessment Form is useful for predicting the duration of nonsurgical endodontic treatment. Among many factors, complicated anatomic features of the treated teeth increased case complexity and extended operating time.

Este caso clínico relata o reparo ósseo após cirurgia paraendodôntica com o uso do agregado trióxido mineral (MTA) e regeneração tecidual guiada (RTG). Paciente procurou a Clínica de Ortodontia da Faculdade COESP apresentando vestibularização do elemento 11, mobilidade grau II. Ao exame radiográfico, observou-se uma radiolucência apical, da raiz do dente 13 ao 11 necessitando ser encaminhada para endodontia. Com prognóstico duvidoso, seguiu-se com o retratamento endodôntico e após 3 meses a intervenção cirúrgica foi realizada. Logo após, o enxerto foi acomodado na loja óssea, seguido da adaptação da membrana e sutura. Após 6 meses, não havia sensibilidade à percussão vertical e à palpação em região de fundo de vestíbulo. Após 3 anos observou-se neoformação óssea na região periapical, dessa forma, conclui-se que a conduta escolhida de desinfecção do meio e utilização de enxerto ósseo somado à regeneração tecidual guiada promoveu uma regeneração óssea satisfatória com a consequente manutenção do elemento dentário na cavidade oral.

Objective: The aim of the study was to assessment of technical quality of 9562 endodontic cases treated by heterogeneous groups with different clinical experience. Material and methods: This retrospective study reviewed the dental records of 8590 patients (9562 endodontic cases and 13203 root canals including 3340 retreatment root canals) treated by fourth-year undergraduates, fifth-year undergraduates, endodontic program students, and endodontic specialists between December 2017 and December 2018 at the Department of Endodontics of Necmettin Erbakan University Faculty of Dentistry in Konya. The length, density, and taper of root fillings, the acceptable technical quality of the root filling criteria, and the presence of procedural errors, were recorded. Chi-square tests with a significance level at p=0.05 were used for statistical analysis. Results: Acceptable root fillings were found in 71.5% of endodontic cases. Clinical experience showed statistical differences in the root filling quality among fourth-year undergraduates, fifth-year undergraduates, endodontic program students, and specialists (52, 63.1, 77.9 and 86.5% respectively). Fractured instruments and missed canals were noted in 9.8% and 0.57% of cases treated by endodontic program students significantly more than the other clinical experiences. No statistical differences were found among the clinical experiences in other procedural errors. No statistically significant differences were found between age range and gender in additional root canals. Statistically significant differences were observed between the number of root canal retreatments in terms of age range and gender. Conclusions: Clinical experience affects the technical quality of root canal treatments. Fractured instruments and missed canals, especially by endodontic program students, should be given more attention. KEYWORDS Dental radiography; Education; Undergraduate.
