The relationship between two different measures of osteoarthritis bone pathology, bone marrow lesions and 3D bone shape: data from the Osteoarthritis Initiative

Abstract: SummaryObjectiveBone shape and bone marrow lesions (BMLs) represent different features of Magnetic resonance imaging (MRI)-detected subchondral pathology in osteoarthritis (OA). The aim of this study was to determine how these features are related and how they change in OA progression.Methods600 participants from the Osteoarthritis Initiative (OAI) FNIH Biomarkers Initiative were included, having Kellgren–Lawrence grade 1–3, at baseline and MRI data at baseline and 24 months. The associations between 3D quanti… Show more

“…Bone shape vector, defined as the deviation from normal bone morphology, showed greater responsiveness to change than bone marrow edema lesions. Furthermore, higher bone shape vector for the femur and tibia, indicating greater deviation from normal bone morphology, was associated with the presence of bone marrow edema lesions 48 . Wise et al used statistical shape modeling of knee X-rays to characterize gender-related differences in femur and tibia bone shape in 608 subjects from the OAI.…”

“…BMLs have been associated with pain in OA [ 25 ]. BMLs are also positively correlated to bone shape changes, and bone shape change has shown a higher correlation with OA progression than BMLs [ 30 , 31 ]. Unlike BMLs, which are not seen in every case of OA, we believe that shape changes occur in all cases of OA, and OA patients with abnormal bone shape can be regarded as a special high-risk group.…”

“…With the development of MR sequences and semi-and automatic quantitative determinations of knee tissues, it has become possible to quantify not only the cartilage volume, its loss, average thickness, and morphology [97,98], but also the alteration of the menisci, ligament, infrapatellar fat pad, bone curvature/shape, bone marrow lesions (BMLs), and joint effusion/swelling [99][100][101][102][103][104][105]. All these joint tissues have been found to be involved in the OA pathological process.…”
