The Relationship of Myocardial Collagen Metabolism and Reverse Remodeling after Cardiac Resynchronization Therapy

Abstract: SummaryBackgroundIn the majority of patients with a wide QRS complex and heart failure resistant to optimal medical therapy, cardiac resynchronization therapy (CRT) leads to reverse ventricular remodeling and possibly to changes in cardiac collagen synthesis and degradation. We investigated the relationship of biomarkers of myocardial collagen metabolism and volumetric response to CRT.MethodsWe prospectively studied 46 heart failure patients (mean age 61±9 years, 87% male) who underwent CRT implantation. Plasm… Show more

“…On the other hand, mTOR inhibition is able to induce regression of LVH [11] and cardiac fibrosis [27] induced by pressure overload [25]. The cardiac remodeling is associated with an increase in collagen production [28], being the collagen types I (80%) and III (11%) the most frequent in the heart [29,30]. Accordingly, in our study PINP levels at month-6 were lower in EVR treated patients.…”

“…A summary of comparisons of baseline variables between responders and non-responders is presented in Table 3 (E-component 3). It is to highlight that NICM was more often present in the responder group, with a reported significant statistically difference in eight studies, [9] , [10] , [12] , [14] , [17] , [18] , [19] , [20] as well as a longer QRS duration. [12] , [13] , [15] , [16] , [19] .…”

“…We found no differences in DT and e’ velocity ( Table I ). It is known that the right atrial and RV chamber enlargement in SSc are directly related with the heart failure symptoms and mortality (30, 31, 32, 33, 34, 35, 36, 37). RA size is correlated with clinical outcomes in PAH (28).…”
