The relationship of oleic acid/albumin molar ratio and clinical outcomes in leptospirosis

Abstract: Human leptospirosis is an acute infectious zoonosis presenting specific lipid disorders. Previous in vitro studies showed both leptospira glycolipoprotein endotoxin, and high oleic acid levels were associated with Na/K-ATPase inhibition that is amplified by the reduction of circulating albumin levels. In this study, we aimed to investigate the relationship of oleic acid/albumin (OA/A) molar ratio and clinical outcomes in Leptospirosis. Through a prospective observational cohort study emp… Show more

“…In addition, NEFA can activate toll-like receptors and inhibit Na+/K+-ATPase, causing lung injury and edema (249, 250). Our group showed a relationship between the oleic acid/albumin molar ratio and clinical outcomes in critically ill patients, with a higher ratio indicating higher mortality in these patients (251). Omega-9 treatment increases CPT1A mRNA in the livers of septic mice, reduces plasma NEFA levels, and improves survival and clinical status (105).…”

“…[39,40]. Its main characteristic is its specificity for the Na/K-ATPase pump [41] on the cytosolic surface of cell membranes through the action of its lipid component on the cell membrane (oleic acid), and the poor adsorption of the albumin toward these fatty acids [42][43][44]. Synergistic inhibition reduces the co-transport activity of the Na/K-ATPase pump, increasing affinity for sodium and potassium loss [45,46].…”

“…The relationship of oleic acid/albumin and the reestablishment to near control values was shown in preliminary studies to be an excellent parameter indicating leptospirosis patient recovery, demonstrating its potential as a prognostic biomarker factor for leptospirosis outcomes [ 94 ]. Due to its importance, a new test adapting three different methodologies to provide information concerning albumin–NEFA saturation determined by albumin isoelectric focusing and staining with bromocresol green was developed, providing an alternative to the current methodology for serum NEFA quantification with the advantage of measuring albumin’s ability to bind NEFA [ 95 , 96 ].…”
