The Reliability of the Vestibular Autorotation Test (VAT) in Patients with Dizziness

Abstract: Objective-To establish intrarater and interrater reliability of the Vestibular Autorotation Test (VAT) (Western Systems Research Inc., Pasadena, CA) in a clinical sample of individuals reporting dizziness.Study Design-Ninety-eight patients with reports of dizziness referred for vestibular function testing performed repeated trials of horizontal VAT. A subsample of 49 individuals repeated the test for a second rater.Results-Approximately 66% of subjects were unable to meet the performance criterion of six conse… Show more

“…Both tests depend on the evaluation of VOR. However, VOT is not preferred in clinical practice because it is difficult and has low reproducibility and reliability 16,17 . vestibular autorotation test was compared with the head thrust test, which comprises the basis of vHIT, and head thrust was found to provide more accurate results 18 .…”

“…Although the number of reports on VAT and aging is limited, it is not surprising the two reports differ in their conclusions given the large test variance and poor test-retest reliability that is known to exist with VAT. 113,114 Comprehensive Vestibular Study Maes and colleagues conducted one of the most comprehensive clinical studies to investigate age-related physiologic degeneration. 25 The authors provide a brief summary of the literature, which has been summarized in Table 3.…”

“…Second, the EOG and head movement signals were transmitted wirelessly, thereby significantly reducing the noises from the cables. Third, in comparison with literature review,19,30,31 the variance of cVAT was smaller30 and the test–retest reliability was better by the intra-class correlations of greater than 0.85 and even as high as 0.97 19,31. Finally, existing VATs use gain and phase to represent vestibular function, where the gain is calculated as the ratio of maximal velocity of eye and head movements at that time point.…”

“…However, there were limited reports about the change of vestibular function with aging evaluated by VAT. Besides, VAT is still not widely used clinically because of several weakness such as uncertain sensitivity and specificity, and poor test–retest reliability 19. Other drawbacks of VAT include a slippage of the head velocity sensor at high-frequency rotations during testing, the contributions of cervico–ocular reflex to compensate eye movements, and incompatible results between different VAT systems.…”
