The Reproducibility of the Point of Care Microcirculation (POEM) Score When Used to Assess Critically Ill Patients: A Multicenter Prospective Observational Study

Abstract: Background: The current standard of analyzing microcirculatory video microscopy is time-consuming and occurs away from the patient, limiting its clinical utility. Point-of-care assessment with incident dark field (IDF) microscopy, however, may offer greater clinical applicability. We aimed to determine the reproducibility of the Point of Care Microcirculation (POEM) tool when used at the bedside in critically ill patients. Methods: A multinational, mult… Show more

“…Despite tremendous efforts to improve video quality and standardize reporting, the quantitative analysis of functional measures remains challenging and is often performed manually [6]. Several visual scoring systems and specialized (semi-) automated analysis tools have been developed to shorten time-to-result and improve accuracy [14][15][16][17]. Common to all these approaches is the (automated) selection and pooled analysis of microvessels with a diameter < 20 (rarely < 10) µm.…”

“…Similarly, a rapid subjective categorization of microcirculation as “good,” “bad,” “very bad” may be sensitive and specific enough to identify the presence of microvascular abnormalities ( 39 ). Watchorn et al applied the Point-of-care Microcirculation tool (POEM) score, a five-point score based on visual assessment of overall microcirculatory flow, showing good reproducibility ( 40 ).…”

“…They used the Point-of-care Microcirculation (POEM) score, which is an ordinal scale from 1 (worst) to 5 (best). This score has shown reasonable reproducibility and interobserver reliability [44]. Doing the POEM score at the bedside negates the need for the assessor to leave and assess the microcirculation images elsewhere, increasing clinical utility.…”

“…With the introduction of the AVA software, used by some of the groups in this study, the assessment of images is now objective rather than subjective. Using the POEM score is another way that future studies can increase reliability, reproducibility and usability of the microcirculation at the bedside [44].…”
