This is a study of Political Economy, Law & Economics, and Public Choice, applied to COVID-19 crisis management, and how the Spanish healthcare sector has operated under stressful conditions. Market and state failures are evaluated and some improvements are offered, according to the theories of Austrian Economics and New-Institutional Economics. At the macro level, the premise is the decentralization of the Spanish healthcare system a long time ago, to provide a better service to citizens, according to the idiosyncrasies of the Autonomous Communities (similar to federal states). The crisis has evidenced the failures of the Spanish system and its semi-federal model, without coordination to manage the trouble. Also, the General Government's recentralization attempt has failed too, proving Mises's theorem on the impossibility of economic calculation in intervened and coactive systems, with problems of shortages, lack of coordination, etc.; Buchanan-Tullock's theorem on the unfinished agenda of state interventionist and it suppression of private sector was also proven. At the micro level, health institutions (hospitals and health centers) have fallen into the paradox of media overexposure and the fake-news risk, because the more information they have tried to transmit, the more confusion they have caused, reducing the value of the supposed transparency and accountability, in addition to decreasing citizen wellbeing, giving way to a higher level of dissatisfaction and more risk of a syndemic. To perform the analysis of accountability and wellbeing perceived, this paper has used quantitative contrast techniques on secondary sources, such as the surveys of Centro de Investigaciones Sociológicas (part of the Public Sector) or Merco rankings (independent institution).

Health information and communication are key elements that allow patients and family members to make decisions about end-of-life care and guarantee a death with dignity. Objective: To understand caregivers’ experiences regarding health information and communication during the illness and death of family members. Methods: This qualitative study was conducted in Andalusia based on the paradigm of hermeneutic phenomenology. Participants were caregivers who had accompanied a family member at the end of life for over 2 months and less than 2 years. Five nominal groups and five discussion groups were established, and 41 in-depth interviews with 123 participants were conducted. Atlas.ti 7.0 software was used to analyze the discourses. A comprehensive reading was carried out along with a second reading. The most relevant units of meaning were identified, and the categories were extracted. The categories were then grouped in dimensions and, finally, the contents of each dimension were interpreted and described given the appropriate clarifications. Results: Four dimensions of the dying process emerged: differences in caregivers’ perceptions of information and communication, a conspiracy of silence, consequences of the absence or presence of information, and the need for a culture change. Conclusions: Poor management of health information and communication at the end of life increased the suffering and discomfort of patients and their families. The culture of denying and avoiding death is still present today. A change in education about death would better enable health professionals to care for patients at the end of life.
Background Different studies have shown the importance of corporate reputation, corporate image and corporate identity and how they are present in the healthcare field. These studies show that these terms establish reputation as a socially constructed asset, which has been gaining relevance in organizations due to its increasing importance as a strategic resource. The aim of this study is to analyze the psychometric properties of an instrument used to measure reputation in the banking field, in its Spanish version, adapted to the health sector, thus determining whether it is a reliable and valid instrument to be used in future research on samples of users in the health sector. Material and Methods In a pilot phase, the survey was applied to 97 patients. Then, the restructured instrument was applied to a sample of 323 men and women who attended the Dental Clinic of the Andrés Bello University, Concepción (Chile). During the pilot phase and application of the instrument, two exploratory factor analyses, Bartlett’s test, the Kaiser-Meyer-Olkin coefficient (KMO), the Principal Component Analysis (PCA) and the Varimax method with Kaiser normalization were applied. During the applicability analysis phase of the instrument, two confirmatory factor analyses were also applied for Relational Commitment (RC), Behavioral Intention (BI) and Relational Value (RV). Results According to the results obtained, the instrument initially created by Khan, Kadir and Wahab for the banking area can be considered reliable and solid for estimating reputation in the health area. The adaptation made to the instrument is considered to exhibit appropriate psychometric properties consistent with the original instrument. Conclusions Measuring relational value can go a long way in determining the future behavior of users who are accustomed to visiting an organization associated with the health sector.
