Aim:The purpose of the present study was to comparatively evaluate the effect of presence of a 2 mm ferrule and different type of dowels on fracture resistance of mandibular premolars.Materials and Methods:Fifty uniradicular mandibular premolars were divided into five groups (n = 10). Ten teeth received no treatment (group I). Samples in group II & III were decoronated 2 mm above cemento-enamel junction and received custom cast dowel-core and fiber dowel-composite core respectively, with 2 mm ferrule. Samples in group IV & V were decoronated at CEJ and were restored using cast dowels and fiber dowel-composite cores, without any ferrule. The restored teeth received metal ceramic crowns and were mechanically loaded. The specimens were subjected to a static load, until fracture, to determine the fracture resistance and fracture mode.Results:The samples with 2 mm ferrule had a higher fracture resistance than non ferrule groups. Within non ferrule groups, there were no significant differences in the fracture resistance. Specimen restored with cast dowel had more incidence of non-repairable fracture.Conclusions:Presence of ferrule increased the fracture resistance of endodontically treated teeth. In case of absence of ferrule, fiber dowels had similar fracture resistance as that of cast dowels and showed increased incidence of repairable fracture.

The best plan for success is to begin with the end in mind. It generally is agreed that the successful treatment of a badly broken down tooth with pulpal disease depends not only good endodontic therapy, but also on good prosthetic reconstruction of the tooth after endodontic therapy. The primary purpose of the post is to retain a core that can be used to retain definitive prostheses. A post and core help prevent coronal fractures when the remaining coronal structure is very thin after tooth preparation. When a substantial amount of coronal structure is lost a cast post and core is indicated. In this clinical report the root canal treated left mandibular first molar is restored with split cast metal post and core design using the direct technique for the fabrication of the post and core pattern using the autopolymerizing acrylic resin material and then further casted in the laboratory using the base metal alloy.

ÖZET AMAÇ: Kök dentini ve fiber postlara uygulanan farklı yüzey işlemleri sonrası, push-out testi ile bağlanma dayanımının değerlendirilmesi. YÖNTEM: Çalışmada çekilmiş altmışdört adet tek kanallı alt çene küçük azı dişleri kullanıldı. Örnekler kök yüzeyine EDTA kullanımlarına göre iki ana gruba ayrıldı. Her iki ana grup kendi içinde fiber post yüzeylerine uygulanan işlemlere göre (%9,6'lık hidroflorik asit, silanizasyon) dörder alt gruba ayrıldı. Ardından tüm örneklerin apikal, orta ve servikal bölgelerinden 1,5 mm kalınlığında kesitler alınarak itme testi ile bağlanma dayanımları ölçüldü. BULGULAR: En yüksek bağlanma direnci örneklerin koronal üçlüsünde, en düşük bağlanma direnci ise apikal üçlüde görüldü. EDTA kullanımının etkisi değerlendirildiğinde, smear tabakası kaldırılan grup istatiksel olarak anlamlı derecede daha yüksek bağlanma direnci gösterdi. En yüksek bağlanma direnci tüm yüzeylere işlem uygulanan grupta görüldü (p<0,05). En az bağlanma direnci ise kontrol grubunda bulundu (p<0,05). SONUÇ: EDTA, hidroflorik asit ve silan uygulaması, fiber postların kök dentinine bağlanma direncini arttırmaktadır. Anahtar Kelimeler: post-core sistemleri, bağlanma direnci, push-out testi
