The Risk of Immunosuppression: A Case of Salmonella Meningitis

Abstract: Salmonella meningitis is a rare infection, particularly in adults. We report the case of a 75-year-old female with a history of rheumatoid arthritis on TNF-antagonist immunosuppressive therapy who initially presented to the hospital for management of back and leg pain and was ultimately diagnosed with bacterial meningitis secondary to Salmonella species infection. She was treated with ceftriaxone with slow improvement in neurological function. Though the source of infection was never clearly identified from mu… Show more

“…Salmonella clearance is dependent upon macrophage phagocytosis, and in particular tumor necrosis factor inhibitors prevent activation and proliferation of macrophages. Consequently, this diminished activation of an inflammatory response leads to increased risk of disseminated Salmonellosis [ 6 , 9 ]. The identification of this patients DMARD therapy including tumor necrosis factor, adalimumab, as a key contributing factor for his pleuropulmonary salmonellosis was crucial.…”

“…These focal infections are most commonly seen in immunocompromised patients, i.e. HIV positive patients [5] or patients having immune suppressing medication like steroids [7] or tumor necrosis factor antagonist [8] or a previously presumed immunocompetent patient later found as immunosuppression, i.e. AIDS [9].…”

“…But, when compared with other Gram-negative bacteria, nontyphoidal Salmonella is a less common etiologic factor for septic arthritis [7] . However, underlying states such as malignancy, hemoglobinopathy, diabetes, human immunodeficiency virus (HIV) infection, surgical operations, and other immune suppressive conditions predispose a patients to Salmonella arthritis [8] . In this report, we describe a case of bilateral hip septic arthritis due to nontyphoidal Salmonella after COVID-19 in a young patient with acute lymphocytic leukemia (ALL).…”
