The Roadmap of Colorectal Cancer Screening

Abstract: Colorectal cancer (CRC) is the third most common form of cancer in terms of incidence and the second in terms of mortality worldwide. CRC develops over several years, thus highlighting the importance of early diagnosis. National screening programs based on fecal occult blood tests and subsequent colonoscopy have reduced the incidence and mortality, however improvements are needed since the participation rate remains low and the tests present a high number of false positive results. This review provides an over… Show more

“…FOBT and FIT have low sensitivity in pre-neoplastic lesion detection and a high false positive detection rate. Furthermore, colonoscopy is an invasive procedure with poor compliance among those eligible for CRC screening [ 2 , 3 ]. Therefore, there is a need for new reliable biomarkers, which can predict risks of early recurrence and metastasis in CRC patients, biomarkers that could be easily incorporated into the routine diagnostic workup [ 4 ].…”

“…It, therefore, may remain a primary screening procedure for CRC. However, other research directions pursue raising sensitivity and specificity of currently available alternative screening procedures, such as the imagistic analysis [ 8 ], epigenetic faecal biomarkers [ 9 ], urine, and exhaled breath or blood-based biomarkers [ 10 ]. In Europe, Colon Capsule Endoscopy is currently considered acceptable as a screening method regarding specificity and sensitivity.…”

“…27,28 The favourable trends in colorectal cancer are largely attributable to progress in early diagnosis, screening, treatment and management of the disease. 29,30 The less favourable rates and trends in Poland likely reflect lesser and later adoption of these protocols there, and probably in other countries of central and eastern Europe. The consistently lower rates and more favourable trends in women from cohorts born after the 1930s suggest a possible favourable effect of female hormones.…”
