The role of data and safety monitoring boards in implementation trials: When are they justified?

Abstract: AbstractThe National Institutes of Health requires data and safety monitoring boards (DSMBs) for all phase III clinical trials. The National Heart, Lung and Blood Institute requires DSMBs for all clinical trials involving more than one site and those involving cooperative agreements and contracts. These policies have resulted in the establishment of DSMBs for many implementation trials, with little consideration regarding the appropriateness of DSMBs and/or key adaptations need… Show more

“…Pragmatic and implementation randomized clinical trials have natural areas of overlap. For example, multisite pragmatic and implementation trials grapple with similar regulatory issues [ 10 12 ]. Pragmatic and implementation trials may be designed to target both individual patient effectiveness and provider-level practice change outcomes [ 5 , 7 , 8 , 13 – 15 ].…”

“…13 describe practice facilitation as an important implementation strategy to communicate shared project goals and monitor practice behaviors, while Sterling et al, 15 examine the role of a specific group of neglected stakeholdershome care workers. Fiscella et al 16 discussed the role of data and safety monitoring boards (DSMBs) in implementations trials, raising the question about whether monitoring for differences in effectiveness is necessary or appropriate, and whether additional systems-level implementation outcomes should be monitored, such as impact on workflow and workforce that could create strain on the staff or increase access to care barriers.…”

“…Fiscella, et al [ 6 ] have argued that data safety monitoring boards (DSMB) may have limited applicability for implementation studies, as NIH does not generally require DSMBs for minimal risk trials. Further, typical implementation study features (e.g.…”
