The role of duplex sonography and angiography in the investigation of carotid artery disease

Abstract: Some patients with symptomatic carotid stenoses of greater than 70 % benefit from carotid endarterectomy. This study was designed to compare the accuracy of duplex ultrasound with angiography in assessing the degree of carotid stenosis in 73 patients with symptoms of recent carotid territory ischaemia. Ultrasound was found to be most accurate in the group of patients with normal vessels or mild stenoses (0-29 %) when there was 90 % concordance between ultrasound and angiography. Ultrasound was found to be leas… Show more

“…One of the US data points appears to be an outlier. 121 This study measured stenosis using the ECST method, 72 whereas the other papers all used the NASCET method. 93,104,120 There were too few data in the ROC plot for 50-69% stenosis to say whether the points lay on a curve or not ( Figure 14).…”

“…There is some heterogeneity present in the 0-49% or occluded meta-analyses, especially the US specificity data, according to the VIF. There are four studies contributing to that meta-analysis, and one of them gives a noticeably different estimate to the other three, 121 which may, as discussed above, be an outlier.…”

“…1. According to the classification criteria of carotid and vertebral artery stenosis degree made by North American Symptomatic Carotid Endarterectomy Trial (NASCET) [5], three grades are defined: mild (0-29%), medium (30%-69%) and severe (70%-99%). So, the patient's right side vertebral artery beginning part had severe stenosis, which was 10mm long, and the surface of atheromatous plaque was smooth without anabrosis.…”

“…Therefore, early diagnosing and efficient treating are important to reduce the incidence of ischemic stroke. One ideal method to diagnose vertebral artery stenosis is the Digital Subtraction Angiography (DSA), which can accurately display the lesion location, the length of the narrow and the degree of stenosis [2]- [5].…”
