The Role of Multimodality Imaging After Cesarean Delivery

Abstract: There has been a global increase in the rate of cesarean delivery during the last 30 years. As a result, many women of reproductive age now have uterine scars. Well-recognized obstetric and gynecologic consequences include cesarean scar pregnancy, scar dehiscence or rupture, and scar endometriosis. More recently, abnormal menstrual bleeding, pelvic pain, and secondary infertility have been reported as a consequence of cesarean delivery. This article reviews the range of normal imaging findings after cesarean d… Show more

“…The fibrous component will show low intensity on t2 weighted images. 18 CT scan of the lesion will show soft tissue lesion involving rectus sheath. 19 The appearances in CT and MRI would depend on the phase of menstrual cycle, the proportion of stromal and glandular elements, amount of bleeding and surrounding inflammatory response.…”

“…2 MRI is a non-invasive modality which allows good tissue characterisation and offers the advantage of multiplanar evaluation of the lesions. 3,16 It provides an accurate assessment of the extent including depth of involvement (i.e. whether limited to the subcutaneous plane, intramuscular plane, peritoneal involvement etc.)…”

“…Isolated T2 hyperintense foci, as observed in case 2 may also be seen, which represent ectopic glandular elements. 16,17 Diffusion characteristics may vary depending on the predominant tissue within the lesion. On post-contrast images, the lesions invariably demonstrate intense enhancement (evident in all four cases) with an occasional feeding vessel sign, which has been attributed to the associated extensive inflammation.…”

“…In addition to the anamnesis and clinical examination, the ultrasound should be the first-line imagistic method for diagnosis. High-frequency linear transducers are best for such assessment [13].…”
