The Role of Praziquantel in the Prevention and Treatment of Fibrosis Associated with Schistosomiasis: A Review

Abstract: Schistosomiasis remains a major global public health concern. Currently, the control of this neglected tropical disease still depends on chemotherapy to reduce the prevalence and intensity of the parasite infection. It has been widely accepted that praziquantel is highly effective against all species of Schistosoma, and this agent is virtually the only drug of choice for the treatment of human schistosomiasis. Mass drug administration (MDA) with praziquantel has been shown to be effective in greatly reducing t… Show more

“…In this regard, a recent study highlighted that presumptive praziquantel treatment of all immigrants from endemic countries was an effective alternative in terms of costs, survival, and sequelae [35,36]. However, the efficacy of praziquantel in chronic schistosomiasis with fibrotic lesions is uncertain [37][38][39]. MGS treatment studies with praziquantel have only been carried out in endemic countries, and in acute infections [22,40,41].…”

“…The most serious, intractable and sometimes fatal complication is granulomatosis and fibrosis of the liver caused by the embolized eggs as they are ectopically matured to embryonation [2][3][4]. Although the current control measures based on mass drug (praziquantel) administration are effective in killing adult schistosome worms [17,18], the sustained pathology due to mature eggs that have already been laid cannot be overlooked [19,20]. Hence, there is an urgent need to develop any egg-targeting strategies against pathology induced by schistosome infection.…”

“…Although chemotherapy successfully eradicates adult worms and inhibits the accumulation of schistosome eggs, less effective medications are focused on treating hepatic fibrosis that has already developed, particularly in the chronic and severe phases of the illness. In recent years, significant efforts have been made to find medications that target hepatic fibrosis in schistosomiasis (Niu et al 2022).…”
