The role of procalcitonin in reducing antibiotics across the surgical pathway

Abstract: Procalcitonin (PCT) is widely considered as a highly sensitive biomarker of bacterial infection, offering general and emergency surgeons a key tool in the management of surgical infections. A multidisciplinary task force of experts met in Bologna, Italy, on April 4, 2019, to clarify the key issues in the use of PCT across the surgical pathway. The panelists presented the statements developed for each of the main questions regarding the use of PCT across the surgical pathway. An agreement on the statements was … Show more

“…Procalcitonin is a protein consisting of 116 amino acids and is the peptide precursor of calcitonin ( 15 ). In normal physiological conditions, PCT is synthesized mainly in thyroid C cells and to a lesser extent by neuroendocrine tissues in other organs such as the lung and gastrointestinal tract ( 16 ). In response to stimulation induced by glucocorticoids, calcitonin gene-related peptides, glucagon, gastrin or β-adrenergic signals, PCT is converted to calcitonin before entering the circulatory system and therefore exhibits very low serum PCT levels (< 0.02 ng/mL) under normal physiological conditions ( 17 ).…”

“…Furthermore, several randomized controlled trials evaluated the use of procalcitonin to guide antimicrobial duration in septic patients [ 81 , 82 , 83 , 84 ]. According to a multidisciplinary task force of experts, procalcitonin may be considered as an excellent parameter for guiding treatment and duration of antibiotic therapy in general and emergency surgery [ 85 ]. Further, the use of an early warning score, namely quick SOFA (qSOFA) [ 41 , 79 , 86 , 87 , 88 ], SIRS [ 89 , 90 , 91 ], Modified Early Warning Score (MEWS), and National Early Warning Score (NEWS) [ 92 ], can facilitate the stratification of risk in patients with suspected infection.…”

“…11 Procalcitonin (PCT) in normal condition is present in C-cells of the thyroid gland 12 but in bacterial infection procalcitonin levels are raised in blood and it is one of the sensitive marker of bacterial infection. 13 PCT is involved in apoptosis and facilitate cell damage. It is expensive and not available in all health related facilities.…”
