The role of quantitative cultures of non-bronchoscopic samples in ventilator associated pneumonia

Abstract: Purpose: The objective of this study is to determine the role of quantitative cultures of non-bronchoscopic samples such as blinded bronchial sampling (BBS) and endotracheal aspirates (ETA) in the management of ventilator associated pneumonia (VAP). The study also evaluates the clinical diagnosis of VAP based on the inclusion of Gram stain results of BBS/ETA samples into modified clinical pulmonary infection score (CPIS). Methods: Fifteen out of the 120 patients admitted to respiratory intensive care unit unde… Show more

“…The incidence of VAP in our study was 45.4%, which is bit higher than other previous studies. [131520] But the study by Rajasekhar et al [10] shows the incidence was much higher and the number of study people was also less in their study. The total number of cases in our study and the study duration also was less comparatively, which may reflect the higher incidence of VAP in our MICU.…”

“…The major difficulty of this approach is in obtaining samples from the lower respiratory tract - mainly because of its probable contamination with the upper airway flora, which may result in misinterpretation of cultures. [10]…”

“…The results of the Gram's stain were obtained within the first hour and quantitative cultures were performed immediately as proceeded by Rajashekar and co-workers. [2]…”

“…For definite diagnosis of VAP, 10 5 CFU/ mLwas considered as threshold. [2] Growth of any organism below the threshold was assumed to be due to colonization or contamination. Any growth was characterized by colony morphology and Gram's staining from the plates.…”

“…There is hardly any microbiology laboratory in India that routinely performs quantitative cultures, and quantitative cultures are considered more of a research tool. [292] The bacteriological strategy is considerably more expensive in terms of sampling and diagnostics, but may reduce the overall cost of treatment as fewer patients (only microbiologically confirmed pneumonia) are treated with targeted antibiotic therapy.…”
