The Role of Systemic Retinoids in the Treatment of Cutaneous T-Cell Lymphoma

Abstract: Retinoids are natural and synthetic vitamin A analogs with effects on cell proliferation, differentiation, and apoptosis. They have significant activity in hematologic malignancies and have been studied extensively in cutaneous T-cell lymphoma. Retinoids bind to nuclear receptors and exert their effects through moderation of gene expression. Retinoic acid receptor and retinoic X receptor exert regulatory activity in vivo, binding to distinct ligands. Studies investigating systemic retinoids as monotherapy and … Show more

“…Isotretinoin at doses of 1–3 mg/kg/day achieved ORR of 43–100% and CR <20% with short duration of response (3–15 months) in early MF . Etretinate , no longer available, was studied in the treatment of MF with similar efficacy rates noted.…”

“…SDT, including topical steroids, PUVA, UVB, radiation and topical chemotherapies are used for localized disease control while systemic therapies are reserved for both limited‐stage disease that is poorly controlled with SDT alone and advanced‐stage disease. Systemic treatment options that have been utilized in MF/SS include retinoids , interferon alpha , single agent or combination chemotherapy , HDAC inhibitors , and antibody‐based therapies . For SS, extracorporeal photophoresis (ECP) is frequently used in the front‐line setting .…”
