The role of the microbiology laboratory in the investigation of child sexual abuse

Abstract: Detection of a sexually transmitted pathogen in a child is suggestive of sexual abuse. Consequently, there are very strong clinical, social and legal reasons for diagnosing sexually transmitted disease in children correctly. Carefully considered protocols should be established for all stages of the microbiological investigation. All procedures, from specimen collection to report generation, should be undertaken to the highest possible standard with appropriate documentation. For the more commonly identified se… Show more

“…Specimens from the paediatric gynaecology clinic were supported by a "chain of evidence"-a record of handling specimens from the time of collection to issue of the final report. 7 They were cultured on the following media: chocolate (heated blood) agar (for H influenzae) incubated in 5% CO 2 for 24 hours, blood agar (7% horse blood) incubated anaerobically for 24 hours (for streptococci), selective medium for N gonorrhoeae (brain heart infusion agar with lysed horse blood and supplemented with vancomycin, colistin sulfate, nystatin, trimethoprim), and Saboraud's agar (for candida). A wet preparation was also examined by direct microscopy for trichomonas.…”

“…Indeed, the finding of a sexually transmitted organism, at a minimum, has important public health implications and perhaps is indicative of criminal activity. Dyson and Hosein published that detection of a sexually transmitted pathogen from a child is suggestive of sexual abuse and as such the diagnosis must be correct for clinical, social and legal reasons [40]. At the time of publication, the 'gold standard' of diagnosis was in vitro culture of the organism as the method offers maximum specificity; however, culture must be followed by appropriate confirmation testing.…”
