Abstract:This inclusion method of pulmonary autograft implantation leads to minimal increases in aortic root size over time, with no reoperations for aortic root dilatation and a low requirement for aortic valve reoperation. The Ross procedure deserves to remain on the surgical menu for aortic valve replacement.
“…Performed as a free-standing graft, a quarter of the pulmonary autograft fail within 2 decades imposing the need for a mechanical valve replacement (8). The inclusion techniques, which can only seldom be made in the children and young adolescents, may provide better results (9). An inclusion in a graft may be performed in adults only and it may be that there is a better age to perform the Ross procedure.…”

“…Therefore, an inclusion cylinder technique with a native aorta or a synthetic external support, such as Dacron, might stabilize the autograft root and improve long-term outcomes (39,59).…”
“…Such data play a large role in the observation that patients undergoing the Ross procedure at experienced centers have the same life expectancy as age-and sex-matched control subjects from the general population. 2,3 Recent evidence suggest that patients undergoing the Ross procedure may even have better long-term survival than those undergoing mechanical AVR. 4 Several important studies have been published in the last few years demonstrating very good to excellent long-term results of the Ross procedure in large series of young adult patients needing AVR.…”
Central Message The Ross procedure is associated with excellent long-term results in centers of expertise. Future versions of valve guidelines should more strongly recommend this option.
