BACKGROUND:To compare the clinical, biochemical, and histopathological features of patients who underwent appendectomy due to a presumed diagnosis of acute appendicitis (AAp). METHODS:The demographic, biochemical and histopathological data of 8206 patients who underwent appendectomy for AAp between January 2006 and March 2014 were retrospectively analyzed in this study. Patients were compared regarding the following characteristics: disruption by season (autumn vs. winter vs. spring vs. summer), working days (weekdays vs. weekends), histopathological findings (AAp vs. normal appendix [NAp]) and histopathological subgroup (non-perforated AAp vs. perforated AAp vs. NAp). RESULTS:Of the 8206 patients aged between 16 and 89 years, 4763 (58.0%) were male. Appendectomy distribution by season was as follows: autumn (n=1959; 23.9%), winter (n=2062; 25.1%), spring (n=2061; 25.1%) and summer (n=2124, 25.9%). NAp rates were higher in summer than those in other seasons. White blood cell (WBC) and neutrophil levels were significantly higher in autumn and winter compared with those in other seasons. In total, 6120 (74.6%) appendectomies occurred on weekdays and 2086 (25.4%) on weekends. WBC and neutrophil levels were significantly higher on weekends than those on weekdays. Appendectomy distribution by histopathological groups as follows: AAp (n=7414; 90.3%) and NAp (n=792; 9.7%). Appendectomy distribution by histopathological subgroups was as follows: non-perforated AAp (n=6966; 84.9%), perforated AAp (n=448; 5.5%), and NAp (n=792; 9.7%). WBC, neutrophil, and TBil levels in the non-perforated and perforated AAp groups were significantly higher than in the NAp group. While most of the patients with perforated AAp (62.1%) and non-perforated AAp (59.6%) were males, most of the patients with NAp (58.1%) were females. CONCLUSION:This study suggests that a relationship exists between demographic features, histopathological findings of appendectomy specimens, seasons, days of the week, and working days in patients undergoing appendectomy.

The most common cause of acute abdomen requiring surgical intervention globally is acute appendicitis (AA). In this study, we aimed to investigate how AA diagnosis is affected by seasonal changes in Central Anatolia. Materials and Methods: In this study, patients who underwent appendectomy with the diagnosis of (AA) between January 1, 2015 and January 1,2019 were evaluated retrospectively. We divided the patients into three groups based on their histopathological results: Group 1 Acute Appendicitis (AA), Group 2 Appendix vermiformis (AV) and Group 3 Perforated appendicitis (PA). Patients were evaluated concerning age, sex and season of operation. Results: Seven hundred ninety-five patients participated in our study. They were separated into the following groups: Group 1 (n=614), Group 2 (n=125), and Group 3 (n=56). Concerning the sex variable, no statistically significant differences were found between the groups (p=0.061). The mean age of the patients in the perforated appendicitis group was higher when compared to the other groups (p=0.001). When evaluated by season, it was seen that during winter, the patients in Group 1 were significantly higher than the patients in the other groups (p=0.009). In our study, acute appendicitis was most frequently diagnosed during winter (28.7%); appendix vermiformis was most frequently diagnosed during spring (32%) and perforated appendicitis was most frequently diagnosed during summer (41%). Conclusion: Although the diagnosis of acute appendicitis varies seasonally, age and seasonal changes may be effective in the frequency of acute appendicitis. In Central Anatolia, the incidence of acute appendicitis increased during winter, while the rate of perforated appendicitis increased in summer and the rate of negative appendectomy increased in spring.

Nevi şahsına münhasır bir düşünür olan Fahreddin er- Râzî, disiplinler arası özgün düşünceleri ile ön plana çıkmıştır. O, öğrenmeyi transkripsiyon bir gereklilik olarak görmüş, keyfiyet ve kemiyetine bakmaksızın birçok alanda eser telif etmiştir. Râzî’nin en fazla iştigal ettiği disiplinlerin başında kelam, felsefe ve tasavvuf gelmektedir. Kelam yaparken felsefî bahisleri araştırmış meramını anlatmak için de tasavvuf literatüründen yararlanmıştır. Yaşadığı bu değişim ve dönüşüm hem düşüncelerine hem de eserlerine yansımıştır. Râzî’nin yaşadığı bu düşünsel gelgitler, farklı şekillerde yorumlanmıştır. Kimine göre birer sentez, kimine göre de hakikati keşfetme arayışıdır. Çalışmamızda Râzî’nin düşünsel gelgitleri üç ana başlık altında incelenmiştir. Birincisi kelamcı Râzî’dir. Düşünce hayatına kelam ile başlayan Râzî, ilgi alanları genişleyip kelam nosyonu bir üst düzeye ulaşınca dek kelamcı kimliğini korumuştur. Felsefe ile tanışması sonrasında kelama felsefi bahisleri katarak felsefî kelam devrini başlatmıştır. Artık birçok kelamî konu felsefî zaviyeden tahlil edilir olmuştur. Çalışmamızda Râzî’nin kelamcı kimliği, kelamcı Râzî başlığı altında araştırılmıştır. İkincisi felsefeci Râzî’dir. Râzî’nin felsefeciliği, İbn Sînâ ve meşşâî felsefe ile tanışması neticesinde oluşan bir inşadır. O bu süreçte kelamdan çok felsefe yapmış, kelama da felsefî bir hüviyet kazandırmıştır. Felsefesi daha çok tahlil, tenkit ve açılıma dayalıdır. Râzî’nin felsefeci yönü, felsefeci Râzî başlığı altında irdelenmiştir. Üçüncüsü tasavvufçu Râzî’dir. Kelamcı Râzî’nin tasavvufla ilişkisi araştırılmaya değer bir konudur. Zira her iki düşünce tarzı hem epistemolojik hem de ontolojik açıdan birbirinden oldukça farklıdır. Var olan metodolojik farktan dolayı birçok kelamcı tasavvufa, tasavvufçu da kelama uzak durmuştur. Oysa Râzî kelâmî problemleri temellendirirken tasavvuftan yararlanmayı ihmal etmemiştir. Râzî’nin tasavvufla ilişkisi tasavvufçu Râzî başlığı altında tartışılmıştır. Neticede her bir disiplin durağında bir yönüyle değişen, gelişen ve daha da yükselen Râzî’nin interdisipliner bir kimliğe sahip olduğu tesbit edilmiştir.
