The significance of Epstein–Barr virus detected in the cerebrospinal fluid of people with HIV infection

Abstract: IntroductionEpstein-Barr virus (EBV) is detected in the cerebrospinal fluid (CSF) in people with HIV infection who develop primary cerebral lymphoma (PCL). However, EBV may also be detected in the CSF of patients without PCL, and here the significance is uncertain. MethodsNinety-eight HIV-positive patients had lumbar punctures performed and polymerase chain reaction (PCR) for EBV was undertaken on the CSF. Thirty-eight patients had non-Hodgkin's lymphoma (NHL), including four with PCL. Sixty patients had a CSF… Show more

“…However, the patient's maculopapular rash, recent mosquito exposure, basal ganglia and thalamic involvement on MRI, as well as her early neutrophilic‐predominant CSF pleocytosis were all more consistent with WNV meningoencephalitis . In this case, MDS provided an alternate diagnostic possibility that fit much better with the patient's acute presentation .…”

“…In our study, 56% of EBV-positive patients were HIV infected. EBV has been reported to be associated with primary CNS lymphomas in HIV-infected persons ( 30 , 31 ), and EBV DNA can be detected in the CSF of 80%–100% of HIV-infected patients with primary CNS lymphomas ( 32 34 ); however, EBV also may be detected in the CSF of 12% of HIV-infected patients with neurologic symptoms but without lymphoma ( 35 ). None of the EBV-positive patients in our study had evidence of CNS lymphoma by contrast computed tomography or MRI of the brain.…”

“…Although the role of EBV in the patient's cerebral syndrome is not entirely clear because of lack of evidence for CNS lymphoma by MRI and CSF analysis, it may well presage the potential development of primary B-cell lymphoma in the future if the immunodeficiency were to progress without treatment and may have provided the opportunity to prevent this complication by controlling HIV infection and restoring the cellular immunity. 12 The later development of mania in this patient adds to the literature of cases of mania as a presenting feature of HIV encephalopathy and points to the interesting role of the encephalopathy as a trigger for the manic syndrome in a person with genetic predisposition to affective illness. 13 ' 14…”
