The silent ductus: Its precursors and its aftermath

Abstract: Prophylactic closure of the patent ductus arteriosus has been recommended as a means of decreasing the morbidity of the very low birth weight neonate. This study was undertaken in order to determine potential risk factors involved in the development of the silent ductus, its impact upon both the early cardiorespiratory symptomatology and the subsequent morbidity of the premature neonate, and finally the potential benefit to be derived from prophylactic closure in this presymptomatic stage. Infants with birth w… Show more

“…The factors determining PDA in the preterm are likely to represent an interplay between immaturity of the ductal constrictive mechanisms and exogenous factors relating to the respiratory disease, and possibly circulating prostaglandins. 15 In eight babies in group 3 the absence of any evidence of constriction would suggest that factors such as high circulating prostaglandins were maintaining ductal patency. In nine babies in group 2 ductal closure failed after initially constricting.…”

“…75,134 If so, closing the ductus cannot be expected to alter the ultimate outcomes of these otherwise unrelated processes. 135 The research agenda should prioritize a search for the identity of and methods for modification of these antecedent or intermediary processes, to develop tools for preventing these adverse outcomes (and, perhaps, persistent PDA, as well).…”
