The simply modified intrascleral fixation using round flange (SMURF) technique for intrascleral intraocular lens fixation

Abstract: We describe a simply modified intrascleral fixation using round flange (SMURF) technique and report the clinical outcomes of the surgery. Forty-one eyes of 41 consecutive patients, with intraocular lens (IOL) dislocation, crystalline lens subluxation, and zonular weakness, who underwent surgery using the SMURF technique were included. The modified technique included the use of a conventional 27-gauge needle, a non-bent needle, oblique sclerotomy, direct threading of the leading haptic, and simple placement of … Show more

“…However, the externalization of the trailing haptic in sutureless intrascleral fixation is still the most difficult step and remains a serious concern for surgeons. 7,10 Therefore, in this study, we describe the facilitated trailing haptic externalization technique to simplify this stage and our results showed that this technique significantly simplified intrascleral fixation and eased all surgical difficulties.…”

“…Although several difficulties of the Yamane technique have been overcome with this technique, threading the trailing haptic into the lumen of the needle remains the most difficult step of the intrascleral fixation technique. 7 This stage may cause significant intraoperative complications as well as prolonged surgery time. Furthermore, it becomes more complex and difficult in cases with a narrow anterior chamber, small pupil, or corneal opacity.…”

“…Kim modification includes direct threading of leading haptic into lumen that offers an advantage in eyes with inadequate iris support. Also, it avoids iris injury and pigment dispersion by the tip of the haptic 14 .…”

“…Penggunaan enzim proteolitik berpotensi melisiskan protein penyusun katarak. Pemilihan 30-gauge needle untuk menginjeksikan proteolitik dan pemasangan Intraocular lens (IOL) hanya membutuhkan insisi kecil sehingga dapat menurunkan angka kejadian komplikasi dan meningkatkan angka visual outcome (Kim & Kim, 2021).…”
