The spectrum of right ventricular involvement in inferior wall myocardial infarction: A clinical, hemodynamic and noninvasive study

Abstract: The clinical experience with 37 patients with acute transmural inferior wall myocardial infarction who were assessed for evidence of right ventricular involvement is reported. On the basis of currently accepted hemodynamic criteria, 29 patients (78%) had evidence suggestive of right ventricular infarction. However, only 5 (20%) of 25 patients demonstrated right ventricular uptake of technetium pyrophosphate on scintigraphy. Two-dimensional echocardiography or isotope nuclear angiography, or both, were performe… Show more

“…Since then, several studies have validated the usefulness of volume loading for ischaemic RV dysfunction. [40][41][42][43] Therefore, the initial therapy for hypotension in patients with RV infarction without pulmonary congestion has traditionally been volume expansion, particularly if the estimated central venous pressure was <15 mmHg. 44 In previous studies, maintenance of the RV preload with volume loading and normal saline alone was thought to resolve the accompanying hypotension and improve the cardiac output.…”

“…W przypadku stwierdzenia niedomykalności trójdzielnej można również dokonać pomiaru ciśnienia skurczowego w PK. U chorych z zawałem PK obserwuje się także paradoksalny ruch przegrody międzykomorowej [13,14].…”

“…73 Although highly specific, technetium-99m pyrophosphate scintigraphy is not very sensitive, especially for detecting hemodynamically important RVI. The sensitivity of this method to detect this clinically important presentation of RVI approaches 25%.22 Also, application of this technique is limited in the acute setting because scans should be obtained 24 to 36 h after the onset of the infarct.…”
