This chapter presents the most important features of laser Doppler (LD) techniques: LD flowmetry (LDF) and LD imaging (LDI), together with examples of their clinical applications in dentistry. LDF gives a constant estimation of blood flow at a specified point, whereas LDI gives a 'snapshot' of perfusion at a given point. These methods are non-invasive laser-based techniques for monitoring gingival and pulpal blood flow and could be used as a diagnostic tool. In paediatric dentistry and odontology, LDF proved to be an atraumatic real-time method used for determining the tooth vitality by monitoring the pulp microcirculation in traumatized teeth, fractured teeth and teeth undergoing different conservative treatments (e.g. bleaching, dental preparation for prosthetic restorations, etc.). In periodontology, recent studies showed the ability of LDF to evaluate the health of gingival tissue in different types of periodontal diseases. By using LDF, it is also possible to evaluate the outcome after different periodontal treatments. The laser Doppler line scanning can be used for recording the gingival healing process after a surgical procedure in the anterior area of the oral cavity.

Alterations of the skin microvasculature are known to play an important role in the development and maintenance of psoriatic skin lesions. In this study, we investigated lesional skin in 11 psoriatic patients during a modified Goeckerman treatment using reflectance confocal microscopy (RCM) to study the relationship between clinical clearance and histological normalization of psoriatic skin and the significance of histological abnormalities on the course of disease. The treatment regimen resulted in a significant reduction of the Psoriasis Area and Severity Index (PASI) as well as capillary and papillary diameters (p < 0.0001). The capillary and papillary diameters were still enlarged when compared to those in normal skin (p < 0.001). Capillary and papillary diameters correlated with each other prior to and after treatment (correlation coefficient = 0.63 and 0.64, p = 0.01 and 0.002, respectively) but not with the PASI. Capillary and papillary diameters after treatment and percentage reduction of the PASI during treatment seemed to be better predictors for the clinical course of relapse than the PASI after treatment. These findings make the subclinical changes of psoriatic skin vessels and dermal papillae a legitimate target for treatment. Further investigations of a large group of patients are needed to evaluate the potential of RCM findings as successor of the PASI in the monitoring of psoriasis.

This paper presents a general embedded processing system implemented in a field-programmable gate array providing high frame rate and high accuracy for a laser Doppler blood flow imaging system. The proposed system can achieve a basic frame rate of flow images at 1 frame/second for 256 x 256 images with 1024 fast Fourier transform (FFT) points used in the processing algorithm. Mixed fixed-floating point calculations are utilized to achieve high accuracy but with a reasonable resource usage. The implementation has a root mean square deviation of the relative difference in flow values below 0.1% when compared with a double-precision floating point implementation. The system can contain from one or more processing units to obtain the required frame rate and accuracy. The performance of the system is significantly higher than other methods reported to date. Furthermore, a dedicated field-programmable gate array (FPGA) board has been designed to test the proposed processing system. The board is linked with a laser line scanning system, which uses a 64 x 1 photodetector array. Test results with various operating parameters show that the performance of the new system is better, in terms of noise and imaging speed, than has been previously achieved.
