The Successful Treatment of a Cord Blood Transplant Recipient with Varicella Zoster Virus Meningitis, Radiculitis and Myelitis with Foscarnet

Abstract: Infections of the central nervous system (CNS) with varicella zoster virus (VZV) is a rare occurrence after allogeneic hematopoietic stem cell transplantation. We herein report a case of VZV meningitis, radiculitis and myelitis that developed 8 months after cord blood transplantation, shortly after the cessation of cyclosporine and low-dose acyclovir. Although treatment with acyclovir did not achieve a satisfactory response, the patient was successfully treated with foscarnet. Our report indicates that VZV inf… Show more

“…VZV vasculopathy may also co-manifest in the central nervous system (CNS) as stroke, aneurysm, dissection, and other vascular abnormalities [3 ▪ ]. Several case reports have also demonstrated an association between VZV vasculopathy and prior, subsequent, and/or concurrent viral meningitis or encephalitis [5,6].…”

“…Early radical antiviral treatment of HZ could be helpful to prevent the progress of PHN. However, toxicity and side effects should be considered with long term and high dose of antiviral agents 10 . Thus, it is clinical urgency to seek potential biomarkers to predict PHN accurately for PHN management.…”
