An experimental apparatus for three-dimensional sensitivity mapping of a handheld magnetic probe with a permanent magnet and a Hall sensor was developed. To optimize the shapes and sizes of the magnets, the sensitivity mappings of two types of magnets, column- and cone-type magnets, were evaluated by the experimental apparatus. The longitudinal sensitivities of column and cone types are 8 and 9 mm, respectively, for 5 μL of magnetic nanoparticles. The measured longitudinal sensitivities agree well with the sensitivities calculated by the finite element method. Furthermore, the maximum lateral resolutions of column and cone types are 4.1 and 3.7 mm, respectively. In terms of the directionality, the sensitivities of column and cone types of the angle of 90° with respect to the probe axis fall approximately to 72% and 50% at 6 mm distance from the probe head, indicating that the cone type has high directionality due to its sharp shape. The measurement of sensitivity mapping revealed that the characteristics of the cone-type magnet are superior to that of the column-type magnet for the identification of sentinel lymph nodes.

The ‘standard of care' method for sentinel node mapping is the combination technique using radioisotope and blue dye although some centres use radioisotope or blue dye alone. Radioisotope usage requires licensing, has regulatory issues around handling and disposal of waste, and logistically may be unavailable or difficult to implement in some centres or less developed country. This has led to the development of alternative methods such as superparamagnetic iron oxide (SPIO), fluorescence techniques using indocyanine green (ICG) or fluorescein, computed tomography lymphography, and contrast-enhanced ultrasound scan (CEUS) using microbubbles. The newer techniques will potentially enable a more widespread adoption of this procedure; however, a common barrier for these techniques is the lack of standardisation and no randomised trials to evaluate their effectiveness against the current standard of care. Furthermore, many of these techniques are more costly and may become redundant in node-negative patients with small tumours if ongoing trials show that sentinel node biopsy offers no additional benefit to grey-scale axillary ultrasound. This review discusses the new techniques for sentinel node mapping that have emerged including their pros and cons.

Cel pracyCelem pracy była ocena wielkości sygnału w węźle wartowniczym (WW) w zależności od: budowy piersi według American College of Radiology (ACR), wieku, obecności blizn i czasu od podania znacznika.Materiał i metodyBadaniem objęto 161 procedur biopsji WW. W badanej grupie było 14 chorych z obecnością blizn.WynikiAnalizując wpływ budowy piersi według ACR i czas od podania znacznika do wielkości sygnału w WW, stwierdzono istotną statystycznie dodatnią korelację między wielkoścą sygnału a czasem podania znacznika w grupie chorych z typem budowy 1 i 2 według ACR. W grupie z 3 i 4 typem według ACR nie stwierdzono istotnej korelacji. Wielkość sygnału w WW w stosunku do wieku chorych wykazała istotną statystycznie ujemną korelację wielkości sygnału w WW w całej grupie oraz z ACR 1. Analiza 14 chorych z obecnością blizn wykazała znamiennie statystycznie niższą wielkość sygnału w WW.WnioskiStwierdzono, że czas od podania Sienna+® wpływa na wielkość sygnału w WW w stosunku do budowy piersi według ACR w przypadku typów 1 i 2, a w 3 i 4 brak takiej korelacji. Wydaje się, że u chorych w starszym wieku, a zwłaszcza z typem budowy 1 według ACR, można spodziewać się niższych wartości impulsów w WW. U chorych, u których wcześniej przeprowadzono zabiegi w zakresie kwadrantów górno-zewnętrznych można spodziewać się niższych wartości impulsów w WW.
