The Synovial Prostaglandin System in Chronic Inflammatory Arthritis: Differential Effects of Steroidal and Nonsteroidal Anti‐inflammatory Drugs

Abstract: 1 The present study was undertaken to characterize the spectrum of arachidonic acid metabolites present in synovial effusions of patients with rheumatoid or psoriatic arthritis, and to compare changes in their concentration following a short-term treatment with 6a-methyl-prednisolone (6-MeP: 4-8mg/day) or indoprofen (1.2g/day), a nonsteroidal anti-inflammatory agent with proven synovial prostaglandin inhibitory effect.2 Measurements of prostaglandin E2 (PGE2), thromboxane (TX) B2, 6-keto-PGF1a and PGF2c, were … Show more

“…Similarly, despite the putative role of COX-2-derived 15d-PGJ 2 in the resolution of inflammation (60), no alteration in biosynthesis occurred as symptoms resolved. The efficacy of COX-2 inhibitors and NSAIDs in the treatment of arthritis (61,62) attests to the relevance of COX products in the mediation of pain and inflammation in this disease. Despite the ready detection of PGE 2 in synovial fluid obtained from patients with arthritis who were not taking NSAIDs or related drugs, 15d-PGJ 2 was undetectable.…”

“…Prostaglandins are important lipid mediators that are produced at elevated levels in inflamed tissues including rheumatoid synovium (1,2). PGE 2 and PGI 2 likely contribute to synovial inflammation by increasing local blood flow and potentiating the effects of mediators such as bradykinin and IL-1 that induce vasopermeability (reviewed in reference 3).…”
