The Technique of Endoscopic Exploration for Parathyroid Adenoma of the Neck

Abstract: Background:The technical details of endoscopic exploration of parathyroid adenoma are described here, thereby drawing attention to the enormous potential of this new modality of minimal-access surgery. Methods: Four patients with a parathyroid adenoma that was clearly demonstrated by pre-operative localization imaging techniques were subject to endoscopic exploration using one 11 mm and two 5 mm ports: a technique heretofore undescribed. Results: An adenoma was successfully localized endoscopically in each cas… Show more

“…These endoscopic techniques can be performed by gasless video-assisted methods, such as the minimally invasive video assisted parathyroidectomy (MIVAP) technique 29,30, or completely endoscopically, and through a variety of approaches to including cervical27,28, as well as axillary31, and breast32 approaches.…”

“…Over the past ten years, several different approaches of MIP have been described. These include total endoscopic parathyroidectomy, video-assisted techniques, radio-guided explorations and focused parathyroidectomy using mini-incisions either by a central or lateral approach directly over the adenoma [3,5-8]. All these techniques have considerable advantages and disadvantages.…”

“…Another group reported 3 studies, 1 article on technique development and 2 publications that appeared to relate to the same patients. 20,34,35 Two other groups described technique development, one 36 in dogs and cadavers and the other 37 in dogs and patients. There were 2 reports 21,38 of videoscopic-assisted surgical success in 6 and 39 patients, with a single adenoma.…”

“…Some groups use an endoscopic approach only, 19,34 and the addition of a QPTH assay has occurred in one such major group. Another technique is to localize the unilateral pathological features, approach through a mini incision, and use a lateral probe to provide "superior visualization" through video assistance.…”
