Introduction Soft tissue reconstruction of the hand and distal upper extremity is challenging to preserve the function of the hand as good as possible. Therefore, a thin flap has been shown to be useful. In this retrospective study, we aimed to show the use of the free temporoparietal fascial flap in soft tissue reconstruction of the hand and distal upper extremity. Methods We analysed the outcome of free temporoparietal fascial flaps that were used between the years 2007and 2016 at our institution. Major and minor complications, defect location and donor site morbidity were the main fields of interest. Results 14 patients received a free temporoparietal fascial flap for soft tissue reconstruction of the distal upper extremity. Minor complications were noted in three patients and major complications in two patients. Total flap necrosis occurred in one patient. Conclusion The free temporoparietal fascial flap is a useful tool in reconstructive surgery of the hand and the distal upper extremity with a low donor site morbidity and moderate rates of major and minor complications.

Summary:The osteotemporoparietal fascia flap (OTPFF) has been used for bony defects, especially on the maxilla and orbital floor. However, there are limited reports about the reconstruction of the zygoma. We report the use of composite OTPFF for reconstruction of zygomaticomaxillary complex. The patient had undergone zygomaticomaxillary complex reconstruction with composite OTPFF because of the resection of recurrent postradiation tumor. Extratemporoparietal fascia was harvested and rotated to cover the medullary surface of the bone flap. Flap was successfully transferred with complete bone integration. There were no surgical complications and excellent cosmetic result. The patient is free of disease 12 months post surgery. The OTPFF seems to be a good option in zygomatic reconstruction, even in previously irradiated fields. Utilization of extratemporoparietal fascia to cover the medullary surface of the bone flap has potential to be an advantageous technique to minimize bone exposure and improve flap integration.

Background: Temporoparietal fasciocutaneus (TPFC) and temporoparietal (TPF) pedicle flaps play an important role among many techniques for auricular reconstruction. Because of their unique characteristics, the flaps based on the superficial temporal artery (STA) may be used for various types of reconstruction.Case series presentation: In this paper, we present several applications of the TPFC and TPF pedicle flaps in the reconstruction of defects after ear resection, including the use of the sandwich flap and preparing basis for Vistafix® system.Conclusion: Our experiences presented in this case series confirm the reconstructive potential of the flaps, also in patients in whom the Vistafix® system was used for reconstruction of the ear. In all cases described herein, the use of the pedicle flaps resulted in optimal healing and satisfactory cosmetic and functional outcomes.
