The widespread and popular use of TADs for orthodontic anchoring has become a daily routine in clinical orthodontic treatment. However, as there have been many accidents over the past decade, safety action is needed to help reduce these accidents. We have advocated using the surgical guide and developed a procedure that can benefit patients and orthodontists. The first part of this paper is about the rationale for making the surgical guide for various implant placements that were observed. Due to their anatomical structure, some placements may require particular attention, which is focused on and discussed in length. The second part deals with fabricating the surgical guide in the laboratory procedure. The data from the intraoral SLT acquisition was extracted, and with the DICOM data from CBCT and in a 3-Shapes software, the guidance was designed. The detailed and step-by-step laboratory procedure, CAD/CAM, and 3D printers to make the surgical guide for TADs are explained. The procedure is performed in an easy-to-understand manner to make using the surgical guide possible for the daily practice of orthodontics (Pubmed).

Rationale: Overbite control is a key factor in orthodontic treatment. In some cases, incisor intrusion is essential and could be an optimal strategy for overbite control. The aim of this article was to introduce a biomechanical system called the posterior miniscrew-assisted lever arm, which is innovative in using existing posterior miniscrews to intrude the upper incisors and to control anterior overbite while simultaneously retracting the anterior teeth. Its efficiency in incisor intrusion has been proved with 2 cases.Patient concerns: Two adult women who came for orthodontic treatment with the chief complaint of convex profile were included in this study.Diagnosis: Both patients had similar malocclusions of Class II molar relationship, anterior deep overjet, and anterior deep overbite.Interventions: Their treatment plans were to extract 4 first premolars and insert 2 maxillary posterior buccal miniscrews. After teeth aligning and leveling, en masse retraction was started in both arches. During the space-closing stage, posterior miniscrewassisted lever arms were placed in their upper arches so as to intrude upper incisors and control the overbite.Outcomes: After respectively 4 months and 3 months of incisor intrusion, the anterior overbite was successfully reduced to the normal range in each patient. Cephalometric analysis and superimposition also confirmed the treatment effect of this biomechanical system on incisor intrusion.Lessons: The posterior miniscrew-assisted lever arm is a valuable biomechanical system for intruding incisors and controlling anterior overbite.Abbreviations: d1 = the arm of F1, d2 = the arm of F2, F1 = the intrusive force on the upper incisor, F2 = the extrusive force on the canine, M = moment, U1-PP = the distance from upper incisor to anterior nasal spine-posterior nasal spine plane.

Background: Fragile gingival tissue is a risk factor for the development of gingival recessions. Despite the fact that gingival recessions are more commonly seen around anterior mandibular teeth, previous research has predominantly concentrated on the gingival dimensions in the anterior maxilla. The objective was to systematically compare buccal gingival thicknesses between the upper and lower jaws in individuals with healthy gingival conditions in the aesthetic zone. Methods: A comprehensive search of three databases was carried out until October 2023. Gingival thickness differences between the maxilla and mandible were evaluated by calculating the mean differences along with the corresponding 95% confidence interval (CI). Subgroup analysis was conducted based on the measurement area, measurement method, and tooth category. Results: A total of seventeen studies were included in this systematic review. Eleven studies were included in the quantitative analysis. Quantitative analysis comparing gingival thickness around 2100 teeth in the anterior mandible to 2056 teeth in the anterior maxilla revealed a statistically significant thinner buccal gingiva in the mandible (mean difference: 0.16 mm; 95% CI [−0.24, −0.07]; p = 0.0003). Conclusions: The present systematic review revealed a more delicate buccal gingiva in the anterior mandible. However, further scientific validation is required due to the considerable heterogeneity in study design and the potential presence of confounding variables.
