The treatment of dorsal wrist ganglia by radical excision

Abstract: The dorsal wrist ganglion is the commonest benign soft-tissue tumour of the hand. Its treatment has been the subject of discussion for centuries, many methods being accompanied by an unacceptably high recurrence rate. Surgical extirpation gives the most reliable results and success has been said to depend on the identification of an unvarying deep attachment of the ganglion to the scapholunate ligament. A previous study has suggested that if this is excised, there will be no recurrences and no residual symptom… Show more

“…Since the adoption of surgical techniques that include excision of the entire ganglion complex, including cyst, pedicle, and a cuff of the adjacent joint capsule, recurrence rates have improved significantly. Some authors claim recurrence rates for dorsal wrist ganglia as low as 1e5% 3 and as low as 7% for volar wrist ganglia. 9 Generally, higher reported rates of recurrence are attributed to inadequate dissection in which the tortuous duct system located at the joint capsule is not fully excised.…”

“…Thirteen to twenty percent of ganglia are found on volar aspect of wrist, arising via a pedicle from radio scaphoid-scapholunate interval, scaphotrapezial joint, or metacarpotrapezial joint, in that order of frequency. 3 Ganglia arising from a flexor tendon sheath in the hand account for approximately 10%. Occurrence in other joints as well as intraosseus and intratendinous ganglia are much less common (Fig.…”

“…8,9 Open surgical excision of the ganglion cyst has lowered the recurrence rate to 10% to 40%. 2,3 The key to open surgical excision is identification of the stalk and its removal from the base of the scapho-lunate interosseous ligament along with the surrounding capsule. In open surgical excision, the patient is trading a "bump" for a scar.…”

“…1 Open excision of dorsal wrist ganglions has a recurrence rate of 1% to 14%. 6,9,10 Risks of open excision include infection, neuroma, scarring, and keloid formation. Postoperative stiffness, grip weakness, decreased range of motion, and pain may also occur.…”
