The true prognosis of resected distal cholangiocarcinoma

Abstract: This first propensity analysis demonstrated that DCC and PDAC have the same prognosis, linked to the high rate of early recurrence, particularly associated with the need for combined organ resection. J. Surg. Oncol. 2016;113:575-580. © 2016 Wiley Periodicals, Inc.

“…The most common site of distant metastasis was the liver, followed by the peritoneum. An isolated locoregional recurrence was found in only 11·6 per cent of all patients, in agreement with previous findings of 3–28 per cent. Theoretically, locoregional recurrence arises because microscopic disease remains in the area of surgery, and could be avoided by more extensive resection.…”

“…Approximately half of the 213 patients with disease progression developed recurrence within 1 year. This relatively early recurrence might be explained by the fact that the study cohort included a higher proportion of patients with advanced tumours (AJCC stage II or above) compared with previous reports 6,8,10 . A further explanation may be the closer follow-up in this study.…”

“…The only potentially curative treatment of distal cholangiocarcinomas (DCC) is an oncologic pancreaticoduodenectomy (PD), yielding a 5-year overall survival (OS) rate less than 30% and a median OS of 37 months. 1 This prognosis is closely related to local recurrence and metastatic spread, which require an appropriate treatment. 2 Therefore increased understanding of the risk factors of recurrence is fundamental to improve long term outcomes.…”

“…However, some data are contradictory: similar survival for patients with distal cholangiocarcinoma and PDAC was recently reported[89], as well as comparable prognosis for ampullary and extra-ampullary duodenal carcinomas[90]. Even ampullary carcinoma with the same median postoperative survival as PDAC has been described from Denmark[91].…”
