The two layer method does not improve the preservation of porcine kidneys

Abstract: SummaryBackgroundThe Two layer method (TLM) has been extremely successful in the preservation of the pancreas. However, this has not been thoroughly investigated in other organs or in clinically relevant large animal models. The aim of this study was to assess the effects of TLM in a large animal model of kidney preservation.Material/MethodsPorcine kidneys were retrieved after 10 minutes of warm ischaemic injury and flushed with 300 ml UW solution at 4°C. Kidneys were then either placed in University of Wiscon… Show more

“…Several approaches have been investigated to supply oxygen to the organ (reviewed in). The two‐layer preservation method using perfluorocarbons is effective in preserving different organs . Active gaseous oxygenation of the preservation solution by retrograde persufflation is also possible .…”

“…It has been shown that there were no significant differences in ATP content in pancreatic tissue after preservation when comparing TLM to simple cold UW organ preservation [16]. Moreover, a recent study showed that TLM did not improve the preservation condition of porcine kidneys [17]. Therefore there is a clinical need for new methods to ameliorate organ storage conditions.…”

“…Recently it has also been shown that the TLM only oxygenates [15,16] a small volume fraction of the pancreas and therefore does not provide oxygen to all islet cells located in the organ. Moreover, the use of TLM has shown no superiority over conventional organ storage in kidneys [17].…”

“…The use of TLM has shown potential in other organs but has failed to gain much support as the ability of oxygen to penetrate deep into tissue in more densely capsulated organs has been questioned. In the kidney its beneficial effect was found in a rat model, however, when applied in a porcine model the results showed no advantage [121,[124][125][126].…”

“…PFC can also be formulated as an emulsion for continuous perfusion and was applied during early attempts at machine perfusion [126][127][128][129]. However, the instability and adverse effects of the emulsions at that time prevented their continued application [121].…”
