The use of epidural block in acute pancreatitis: A report of eight cases

Abstract: PmWCaEATIalS is a relatively common condition. It has been estimated by Blumenthal and Probstein x that the incidence is about 1 in 600 admissions (0.16 per cent), based on the routine determination of serum amylase. The incidence of acute necrotizing pancreatitis in an autopsy series of 41,333 cases reported by MeWhorter = in 1952 was approximately 4 per thousand (0.4 per cent). The pancreas normally secretes a clear, colourless, and slightly mucoid fluid containing enzymes (trypsin, chymotrypsin, amylase, ma… Show more

“…Of particular interest to anaesthetists is the pain leading to ventilatory disturbances, such as that following upper abdominal surgery and trauma, an interest reflected in the large number of publications on the subject (7,12,13,14,15,21,34,48,59,72,82,83).…”

“…However, we noted a much lower incidence of post-epidural hypotension (6.6%). The earlier high incidence of hypotension may be a result of the empiric large volumes of local anesthetic that were used in contrast to titrated and diluted local anesthetic in our study 24 . Use of titrated dilute concentration of local anesthetic along with opioid to decrease the requirement and judicious fluid and vasopressor administration could help to avoid hypotension.…”

“…The two major concerns could be the associated hypotension and infective neuraxial complications. An earlier reported incidence of hypotension following use of epidural in patients with acute pancreatitis was 37.5% 24 . However, we noted a much lower incidence of post-epidural hypotension (6.6%).…”

“…Among the nonrandomized studies, 1 had moderate risk of bias, 10 whereas 5 had high risk of bias. The risk of bias was mainly from the risk of confounding 5–9 . The overall evidence level was low.…”
