(1) Background: Supraglottic airway devices (SAD) have been used in children and adolescents undergoing adenotonsillectomies under general anesthesia. This systematic review and meta-analysis investigate the safety and efficacy of using SADs when compared to an endotracheal tube (ETT). (2) Methods: After registering with PROSPERO, databases like PubMed, Scopus, OviD, CINAHL, and Cochrane Library were searched using relevant keywords from the year 2000. We used RoB-2 for risk-of-bias assessment, GRADE for assessing the quality of evidence, RevMan 5.2 for qualitative meta-analysis, and trial sequential analysis (TSA) to corroborate the significant findings of meta-analysis. (3) Results: Out of 200 studies, 5 randomized-controlled trials fulfilled inclusion criteria. The quality of evidence was moderate for laryngospasm, low for airway device failure, and very low for recovery time. The incidence of laryngospasm was comparable between SADs and ETT (RR: 0.80, 95% CI-0.36, 1.80, p = 0.59). The incidence of airway device failure was significantly higher with SADs than ETT (RR: 11.29, 95% CI: 2.73, 46.66, p = 0.0008). The postoperative recovery time was significantly less with SADs than with ETT use (MD: −4.33, 95% CI: −5.28, −3.39, p < 0.0001), which was confirmed by the TSA. (4) Conclusions: The results of this review suggests that use of SADs can provide a lesser postoperative recovery time and comparable incidence of laryngospasm, with a higher incidence of failure of SAD when compared to ETT. Use of SAD for pediatric and adolescent adenotonsillectomies should be individualized based on patient characteristics, and on the expertise of the anesthesiologist and the surgeons involved.

Background General anesthesia with intubation is widely used for surgery. The factors responsible for pharyngeal discomfort after intubation need to be identified. This meta-analysis aimed to clarify the relationship between intraoperative use of muscle relaxants and postoperative pharyngeal discomfort in patients, which is important for postoperative perioperative care. Methods Publications in English were manually searched. After evaluating the quality of selected studies, a meta-analysis was conducted using Review Manager 5.4 software and Endnote. Three reviewers independently selected the data from the selected studies. Results In the included literatures, muscle relaxants were used in the experimental group and normal saline was used in the control group. Ten randomized controlled trials (with a total of 1358 patients) were included in this meta-analysis. The risk of pharyngalgia and cough was lower in patients receiving muscle relaxants than in controls (RR = 0.67, 95% CI: 0.53 to 0.86, P = 0.002; RR = 0.053, 95% CI: 0.007 to 0.381, P = 0.004). Risk of sore throat, and hoarseness were not significantly different between those receiving muscle relaxants and controls (RR = 0.92, 95% CI: 0.68 to 1.23, P = 0.65; RR = 1.01, 95% CI: 0.81 to 1.27, P = 0.92). The preoperative first pass success was significantly higher in patients receiving muscle relaxants than in control patients (RR = 0.37, 95% CI: 0.14 to 0.98, P = 0.05). Conclusion The use of muscle relaxants during intubation anesthesia effectively reduced the development of upper respiratory complications, which also ruled out a negative effect of muscle relaxants in some patients with postoperative upper respiratory complications; however, the incidence of postoperative sore throat and hoarseness was quite high. At present, there is no relevant expert consensus indicating which ventilation device is more efficient for general anesthesia. The present literature review and meta-analysis compared the effects of the laryngeal mask airway and endotracheal intubation on airway complications during general anesthesia. The keywords ‘laryngeal mask airway’, ‘endotracheal tube’, ‘tracheal tube’, ‘children’, ‘pediatric’, ‘anesthesia’, ‘randomized controlled trials’ (RCTs) and ‘randomized’ were used to perform the literature search in PubMed. Quality assessment was performed by two reviewers according to domains defined by the Cochrane Collaboration tool. Data extraction, risk of bias assessment and quality of evidence assessment were performed with the Cochrane tool. A total of 16 RCTs were included. The results indicated that the effects of the laryngeal mask airway group on heart rate variability [mean difference=-13.76; 95% CI, -18.19-(-9.33); P<0.00001], the incidence of hypoxemia [odds ratio (OR)=0.52; 95% CI, 0.28-0.97; P=0.04] and the incidence of postoperative cough (OR=0.22; 95% CI, 0.12-0.40; P<0.0001) were significantly lower than those of the endotracheal intubation group. The success rate of one-time implantation in the laryngeal mask airway group was significantly higher than that noted in the endotracheal intubation group (OR=0.20; 95% CI, 0.07-0.59; P=0.003). However, no significant differences were noted between the two groups in bronchospasm, sore throat, mucosal injury, nausea and vomiting and reflux aspiration. In conclusion, the results indicated that laryngeal mask airway application can reduce complications during general anesthesia compared with endotracheal intubation.
