BackgroundIn low resource settings, short, valid and reliable instruments with good high sensitivity and specificity are essential for the screening of depression in antenatal care. A review of published evidence on screening instruments for depression for use in antenatal services in low resource settings was conducted. The aim of this review was to appraise the best available evidence on screening instruments suitable for detecting depression in antenatal care in low resource settings.MethodsSearching, selection, quality assessment, and data abstraction was done by two reviewers. ScienceDirect, CINAHL, MEDLINE, PubMed, SABINET and PsychARTICLES databases were searched using relevant search terms. Retrieved studies were evaluated for relevancy (whether psychometric data were reported) and quality. Data were synthesised and sensitivity and specificity of instruments were pooled using forest plots.ResultsEleven articles were included in the review. The methodological quality ranged from adequate to excellent. The review found 7 tools with varying levels of accuracy, sensitivity and specificity, including the Edinburgh Postnatal Depression Scale, Beck Depression Index, Centre for Epidemiologic Studies Depression Scale 20, Hamilton Rating Scale for Depression, Hopkins Symptoms Checklist-25, Kessler Psychological Distress Scale and Self-Reporting Questionnaire. The Edinburgh Postnatal Depression Scale was most common and had the highest level of accuracy (AUC = .965) and sensitivity.ConclusionThis review suggests that the Edinburgh Postnatal Depression Scale can be a suitable instrument of preference for screening antenatal depression in low resource settings because of the reported level of accuracy, sensitivity and specificity.Prospero registration CRD42015020316.

Introduction: The postpartum period is generally a time of happiness and bonding between mother and the newborn, but complicated pregnancies can alter this. Postpartum depression (PPD) is a common complication of childbirth. Objectives: To determine the effect of neonatal sepsis on PPD of mothers Methods: This was a descriptive study conducted in secondary and tertiary care hospitals in the district of Gampaha. The study population comprised postpartum mothers of neonates who were diagnosed with sepsis (n=236) and those who were not (n=240). Neonatal sepsis was defined by the presence of one of the clinical signs according to the IMCI strategy or culture positive blood or cerebrospinal fluid. The validated Edinburgh Postpartum Depression Scale and an interviewer-administrated questionnaire were used. Multiple logistic regression was applied and adjusted odds ratios (aOR) and 95% confidence intervals (CI) were calculated. Results: The study revealed that among the mothers of sepsis neonates, 185 (78.4%) were depressed. There were 51 (21.2%) mothers also depressed among the mother of non-sepsis babies. The association between PPD and neonatal sepsis was statistically significant (aOR=13.44; 95% CI=8.68, 20.83). The socio-demographic factors, obstetric and neonatal factors were not independently associated with PPD. Conclusions: Postpartum depression among mothers of sepic neonates was higher than in mothers of apparently healthy neonates. It is essential to arrange a screening programme and counselling sessions while in the hospital for all the mothers.

Depression in Teenager Pregnant Women in a Public Hospital in a Northern Mexican City: Prevalence and Correlates

BackgroundVery little is known about prenatal depression in teenagers in Mexico. We determined the prevalence and correlates of prenatal depression in teenager women attending a public hospital in Durango City, Mexico.MethodsWe performed a cross-sectional study to assess depression in 181 teenager pregnant women who attended a public hospital for prenatal care. We used a validated Mexican version of the Edinburg postnatal depression scale (EPDS) to screen depression. Women with EPDS scores suggestive of depression were further examined to confirm depression by a psychiatric evaluation using the DSM-IV criteria. Bivariate and multivariate analyses were used to evaluate the prevalence association with socio-demographic, clinical and psychosocial characteristics of the pregnant women.ResultsOf the 181 teenager pregnant women studied, 61 (33.7%) had EPDS equal to or higher than 8 (range 8 - 23), and 37 of them were confirmed to have prenatal depression by the psychiatric evaluation. The general prevalence of prenatal depression in the teenager pregnant women studied was 20.4%. Of the 37 women with depression, 34 suffered from minor depression and three suffered from major depression. Thus, the prevalence of minor and major depression in the women studied was 18.8% and 1.7%, respectively. Multivariate analysis of the socio-demographic, clinical and psychosocial characteristics of the teenager pregnant women showed that prenatal depression was associated with a previous episode of depression during pregnancy (odds ratio (OR) = 6.12; 95% confidence interval (CI): 1.68 - 22.30; P = 0.006), and borderline associations with big fetal size (OR = 9.9; 95% CI: 0.94 - 104.24; P = 0.05) and family problems (OR = 3.83; 95% CI: 0.99 - 14.84; P = 0.05).ConclusionsResults demonstrate that prenatal depression is common in pregnant teenagers in Durango City, Mexico. The history of an episode of depression during pregnancy should alert physicians for further depression episodes during pregnancy in teenagers. Further research to elucidate the association of prenatal depression with size of the fetus and family problems in pregnant teenagers is needed.
